Part One The High Risk Factors of Post-stroke DepressionObjective:To analyze the high risk factors of post-stroke depression.Methods:238stroke patients who were long-term living in Zhengzhou area were divided into PSD group (110cases) and non-PSD group (128cases). The effects of gender, age, personality characteristic, degree of education, permanent occupation, income, domicile, social support, complications, stroke type, lesion side, lesion site, nerve function and activities of daily living on PSD were analyzed with use of univariate analysis and multivariate Logistic regression analysis.Results:1Univariate analysis showed that the differences of gender, personality characteristic, permanent occupation, social support, complications, lesion side, lesion site, nerve function and activities of daily living between two groups were significant (P<0.05).2Multivariate Logistic regression analysis showed that gender, personality characteristic, social support, lesion side, lesion site, nerve function and activities of daily living were main risk factors which impacted the occur of PSD.Conclusion:Female, introvertion, less social support, lesions in the left brain and frontal lobe, neurological deficits and poor activities of daily living were the high risk factors which promoted the development of PSD. Part Two The Study of Effects and Mechanisms of Jieyuanshen Paricles on Patients with Post-stroke DepressionObjective:To explore effects and possible mechanisms of Jieyuanshen Particles on PSD.Methods:80patients with PSD were randomly divided into treatment group (40cases) and control group (40cases). Patients in two groups were both given conventional stroke treatment groups and fluoxetine, and patients in treatment group were given Jieyuanshen Particles on the basis of control group for8weeks. To compare the efficacies, degrees of depression, degrees of neurological impairment, activites of daily living and the levels of5-HT, NE, TNF-α, IL-1β and BDNF in serum between two groups.Results:1The total effective rate in treatment group was97.5%; the total effective rate in control group was87.5%. The total effective rates between two groups were statistically significant (χ2=10.660, P<0.05).2After treatment, HAMD, SSS scale scores in two groups were significantly decreased and the treatment group was significantly lower than the control group (P<0.05); ADL scale score in two group were significantly increased, and the treatment group was significantly higher than the control group (P<0.05).3After treatment,5-HT and NE levels in two groups were significantly increased (P<0.05) and the treatment group was slightly higher than the control group (P<0.05).4After treatment, TNF-α and IL-1β levels in two groups were obviously decreased (P<0.05) and the treatment group was obviously lower than the control group (P<0.05).5After treatment, BDNF levels in two groups were significantly increased (P<0.05) and the treatment group was slightly higher than the control group. Conclusion:Jieyuanshen Particles could effectively treat PSD, alleviate symptoms of depression, promote recovery of neurological function, improve activities of daily living, which may be associated with increasing the levels of5-HT, NE, BDNF, reducing the levels of TNF-α and IL-1β in serum.
