ObjectivesThe purpose of this study was to compare the differences of simultaneous bilateral total knee arthroplasty(BTKA)and staged bilateral total knee arthroplasty according to perioperative complications, curative effect, hospital stay period and Postoperative blood transfusion volume Postoperative joint function, Then discuss the advantages and disadvantages of simultaIleous BTKA.MethodsBTKA were performed on125patients from2001through2011bv the same team of orthopedic surgeons. Of which50patients were performed by simultaneous BTKA, while75cases were performed by staged total knee arthroplast, Comparison of perioperative complications, curative effect, hospital stay period and Postoperative blood transfusion volume Postoperative joint function,ResultsIncidence of125cases, of which at the same time substitution groups50cases and installment replacement group of75cases were followed up for6to36months, an average of21months. Group (10cases of infection, the incidence of0.08%year-on-year,7cases, bilateral exchange group stage substitution groups (3cases); Non-infectious postoperative pain in2cases, had occurred in the same period substitution groups; In this study a5cases had a poor healing of incision (the same substitution groups in3, installment replacement group2cases), the rate of0.04%,5patients with obesity, including3cases of diabetes mellitus (2at the same time, stage1case);8cases with cardiovascular complications, bilateral substitution groups7cases during this period, the installment replacement group (3cases); Bilateral knee replacement group increase over the same amount of blood transfusion, substitution groups blood transfusion volume4.1+2.4during this period, the blood transfusion volume2.1±1.7in installment replacement group. This group was followed up from6to36months, An average of21months. SimultaIleous BTKA was shorter stay, but SimultaIleous BTKA Increase the perioperative complications, such as deep vein thrombosis, The cardiovascular disorders, pain, iffection. But thest were not significantly different. SimultaIleous BTKA may increase Postoperative blood transfusion volume,The score of KSS was not significantly different.ConclusionsSimultaneous BTKA iS safe and effective for the treatment of severe osteoarthritis of knee joint. The hospital stay are significantly decreased. Functional rehabilitation in bilateral extremities can be performed simultaneously andtherefore increase the efficiency of surgery. However,The operation injury to the patient is more serious, bleeding more, the anesthetic time is longer and the periopertive complications ale more common in simultaneous BTKA. So the general condition of the patients must be evaluated seriously before operation, and we do not recommend simultaneous BTKA for the old patient whose health condition is very poor.
