ObjectiveFor the purpose of evaluating the effectiveness of Dynesys and PLIF treatment, and the effectiveness of Dynesys and Discectomy for Lumbar degenerative disease, clear which kind of surgical method is better. Provide new guidance for the clinical treatment.MethodsRandomized controlled trials (RCTs) and clinical controlled trials (CCTs) were selected from Embase (-2014.01), MEDLINE (-2014.01), Cochrane Library (-2014.01), CBM disc(2014.01), and CNKI(2014.01). Including RCTs and CCTs. Two reviewers with designed extraction from RevMan5.2software implement the data analysis.Results4RCTs and6CCTs were included. The analysis of the results indicates that:1. Compared to PLIF treatment, Dynesys treatment have the low rate of complication [OR=0.32,95%CI (0.11,0.94)].2. The blood loss of Dynesys group was significantly less than the lumbar fusion group for lumbar degenerative disease [MD=-92.23,-95%CI(-107.96,-76.51)].3. The surgery time of dynesys group was significantly shorter than the fusion group [MD=-22.71,-95%CI(-27,-12.42)].4. The clinical effect of lumbar degenerative is no difference in two group.5. The ROM of instrumented level of Dynesys group is bigger than fusion group.6. The ROM of Adjacent segments of Dynesys group is lower than fusion group.7. The clinical effect and blood loss of lumbar degenerative are no difference between Dynesys group and discectomy group.ConclusionsCompared with PLIF treatment, Dynesys treatment group have a low rate of complication, blood loss and operative time, however, in clinical efficacy, there was little difference, its effect is similar to the PLIF. It prevent the degeneration of the instrumented level and the adjacent segments. Compared to Discectomy treatment group, there was no difference in clinical efficacy. Because the lack of the RCT articles on this subject, we need a lot of rigorous design of large randomized controlled study, in order to increase the strength of evidence.
