ObjectiveTo summarize the effective rate of canalith repositioning maneuver, and explore the treatment effectiveness of Brandt-Daroff exercise for cupulolithiasis and patients that was not cured by canalith repositioning maneuver.MethodsMedical records with BPPV diagnosed in Tianjin People’s Hospital from January2013to October2013were analysed retrospectively. History taking including gender, age, duration of disease, induce position, duration of dizziness, past history and so on. Modified Epley maneuver, Semont maneuver, Barbecue rolling maneuver and reverse Epley maneuver were used in treating appreciate canalithiasis. Cupulolithiasis and patients that was not cured by canalith repositioning maneuver received Brandt-Daroff exercise.Multiple-BPPV received corresponding canalith repositioning maneuver combined with Brandt-Daroff exercise.Results1.52patients with canalithiasis of posterior semicircular canal were treated with modified Epley maneuver meanwhile40patients were treated with Semont maneuver.The initial effective rates were84.6%and87.5%respectively,the total effective rates were92.3%and95.0%respectively.We used Χ2test to analyse the effective rate between the two maneuvers,P>0.05,there is no statistical significance.28patients with canalithiasis of horizontal semicircular canal were treated with Barbecue rolling maneuver,the initial and total effective rate were75.0%and89.3%respectively.2patients got complete relieved after the therapy of reverse Epley maneuver,1patients got partly relieved,the effective rate was100%.2.16cases cupulolithiasis was treated with Brandt-Daroff exercise,the number for posterior canal was5,horizontal canal was11.The symptoms all relieved or disappeared after the therapy of Brandt-Daroff exercise from3days to2months,the average remission time was15days.3.The number of canalithiasis that was not cured by canalith repositioning maneuver was9,they all accepted Brandt-Daroff exercise. Stick to exercise,the symptoms all relieved or disappeared form3days to20days,the average remission time was8days.4. We treated the multiple-BPPV with corresponding canalith repositioning maneuver combined with Brandt-Daroff exercise. The symptoms all relieved or disappeared.5. Follow up A total of10cases recurred during follow-up period, the diagnosis of3cases is consistent with their initial diagnosis,7cases was different from their initial diagnosis. The recurrence rate was7.0%(10/143). Conclusions1. Most canalithiasis patients become asymptomatic after appropriate canalith repositioning maneuver according to the involved semicircular canals. Canalith repositioning maneuver is effective and safe in curing canalithiasis.2. We could choose Brandt-Daroff exercise to deal with the patients that was not cured by canalith repositioning maneuver and patients of cupulolithiasis.3. BPPV have a tendency to recurrence, but it is also effective for canalith repositioning maneuver and Brandt-Daroff exercise.
