Objective:Almost all the Ischemic stroke patients come up with more cognitive, sleep and autonomic nervous dysfunction and disorder. Lots of research had been done to study the relation ship between stroke position and such complications, the conclusions are not identical. The main research have been done on cerebral hemisphere, volume of injury, damage from frontal pole distance, cortical or subcortical damage, brain atrophy, and many other aspects. Most of these studies suggested that stroke infarcts areas exist a certain inner link with cognitive dysfunction. This study aims to analysis the correlation between ischemic cerebral position and the status of cognitive function after stroke.Methods:118ischemic patients and the general information were collected, including frontal lobe (n=17), Temporal lobe (n=14), parietal lobe (n=13), occipital lobe (n=10), basal ganglia (n=24), thalamus (n=19), cerebellum (n=11), brain stem (n=10). The general information includes age, sex, and levels of education. Montreal Cognitive Assessment was used to assess the cognitive impairment of ischemic patients. CT/MRI was used to locate the infarction position. After that, correlation analyses were made between the infarction area and the ORT, EF, NAM, MEM, ATT, LANG, ABS. all the results were analyzed by SPSS17.0, chi-square test, t-test, logistic regression analyze and Pearson relative test were used for different data.Result:(1) Comparison of general information of118cases of cerebral infarction patients and the cognitive impairment showed no statistical meaning;(2) the cognitive impairment rates in infarctions of frontal lobe and thalamus were the highest (90%), the second is temporal lobe with a cognitive rate80%, the lowest rates were found in groups of cerebellum and brain stem (30%); a closely related relationship was found between frontal stroke and cognitive impairment, relative cognitive impairments were found in temporal lobe, parietal lobe, occipital lobe, thalamus, basal ganglia, no relationships were found between cognitive impairment and cerebellum stroke or brain stem stroke;(3) Pearson test showed that all MoCA tests were impaired in basal stroke, especially in the tests of EF,NAM,MEM,ATT; in the frontal lobe stroke, impairment was found obviously in tests of ORT,EF,MEM,ATT; in the parietal lobe stroke, impairment was found obviously in tests of EF,MEM,ATT; in the temporal lobe stroke, impairment was found obviously in tests of EF,MEM,LANG; in the occipital lobe stroke and brain stem stroke, impairment was found only in tests of EF; thalamus stroke showed MEM,LANG impairment; LANG impairment was found in cerebellum stroke. Conclusion:the cognitive impairment rates were different in different area of ischemic patients; there are closely relationship between various cognitive impairment and infarction area.
