ObjectiveBy dynamic observation of serum of patients with acute ischemic stroke, the changes of protein levels of tumor necrosis factor-a (TNF-a), interleukin-6(IL-6), we investigate the relations between TNF-a, IL-6and mechanisms of acute cerebral infarction and cerebral infarction site of inflammation injury; By comparing the changes of TNF-a and IL-6levels of treatment group with control group after treatment, assessing the improvement of neurological symptoms and exploring the intervention of Modified Buyang Huanwu on inflammatory response and mechanisms in acute cerebral infarction, we can provide valuable clinical basis for the treatment of patients with acute cerebral infarction.Methods1. Adopt the prospective research methods and comply with the randomized, controlled principle. Select the inpatients in the Guangzhou University of Traditional Chinese Medicine Hospital of Maoming City, neurology from May2013to March2014. The60selected patients with cerebral infarction within72hours and the diagnosis are accord with Various Cerebrovascular Disease Diagnosis in the fourth national conference of Chinese Medical Association on cerebrovascular disease. CT or MRI were completed within72hours to clear responsibility lesions. Simple random number method used, patients were randomly divided into two groups:the treatment group (Modified Buyang Huanwu and conventional treatment program) and the control group (conventional treatment program). Each group is of30people.2. The serum TNF-a and IL-6were collected respectively from two groups of patients on the frist day and the14th day after treatment as well as the symptom scores and NIHSS score. Venous blood was collected from both groups with the use of CL(Chemi Luminescence) assay method for the determination of serum TNF-a and IL-6concentrations. By collecting basic patient information, clinical symptoms and related test results, establishing information database, we use T-test, chi-square test and apply SPSS19.0statistical software for data processing and statistical analysis to explore the impact of Modified Buyang Huanwu on TNF-a and IL-6of acute ischemic stroke, scores of therapeutic effect of symptom and symptoms of neurological deficits as well as the relations between TNF-a, IL-6in patients with acute cerebral infarction site.Results1. There are no difference in average age, gender, average onset time, smoking history, family history, history of basic diseases of the treatment group and control group (P>0.05).2.14days after treatment, the levels of TNF-a, IL-6of the patients in treatment group and control group were markedly reduced (P<0.05), the treatment group more significantly than the control group (P<0.05).3.14days after treatment, the Chinese medicine symptom integral and neural function defect scale went down in patients of both treatment group and control group (P<0.05). The effect in treatment group is better than in the control group(P<0.05).4. Serum TNF-a, IL-6levels are associated with infarction area. The levels of serum TNF-a was the highest in the brain stem infarction, while the highest content of serum IL-6was found in basal nucleue infarction.5. There is certain relevance between the levels of serum TNF-a, IL-6in patients of infarction in acute phase and the change of hs-CRP.Conclusion1. The patients’serum TNF-a, IL-6, hs-CRP levels increase significantly in the acute phase of ischemic stroke (72hours), suggesting that the mediated inflammatory reaction caused by the inflammatory cytokines plays an important role in acute cerebral infarction, hence early sensitive indicators of acute cerebral infarction.2. There is a possible correlation between the patients’ serum TNF-a, IL-6and the part of the cerebral infarction.3. The remarkable decrease of the levels of serum TNF-a and IL-6in treatment group proves that Modified Buyang Huanwu may reduce inflammatory reaction medium, such as TNF-a and IL-6, and inflammation and reperfusion damage, showing a promising prospect for further experimental study and the clinical effect of the treatment on a massive scale.4. Modified Buyang Huanwu can reduce the traditional Chinese medicine symptom integral and NIHSS scores of cerebral infarction in acute phase, alleviate the symptoms of nerve function defect in patients, and improve the clinical curative effect and the prognosis of patients.5. There is certain relevance among the change of hs-CRP, Serum TNF-a and IL-6in the inflammatory response in acute cerebral infarction.
