Background of researchInsomnia, Chinese medicine called "Bu Mei" refers to non-organic sleep variety of psychological and social factors and arousal disorders seriously affect the patient’s quality of life and work. Severe insomnia can decreased immunity, disease and rehabilitation of low capacity, colds and other illnesses aggravated existing or induced illness, such as depression, hypertension, cardiovascular and cerebrovascular diseases. Chinese medicine treatment of insomnia efficacy, can improve sleep quality in patients with fundamentally stable prolonged sleep time, less adverse reactions. Especially acupuncture, acupuncture as a non-drug therapy, because of its simple, convenient, experience, honesty characteristics, clinically has been widely used for the treatment of insomnia, but the presence of complex clinical studies of acupuncture treatment, the acupoints range of diagnostic criteria and efficacy criteria are not standardized, improper use of statistical methods and other issues, the results of their research is still controversial, affecting the promotion and application of acupuncture. In the present study, Yu Mu by pricking blood therapy acupuncture and combine the advantages of both anger scrambling for the treatment of insomnia, heart-shaped, improve the patient’s quality of sleep, relieve associated symptoms, improve quality of life, with far-reaching social significanceObjectiveThe subject was about research the clinical efficacy of Shu Mu points and bleeding therapy and conventional acupuncture, to observe the efficacy of the Shu Mu points with the bleeding therapy to treat insomnia of liver fire disturbing heart syndrome, to investigate the differences in their mechanism of action of insomnia of liver fire disturbing heart syndrome for the clinical treatment of this disease provide evidence.MethodsThe patients with insomnia were selected from Taiwan Cheng Gong Ji Shi Chinese Medicine Clinic during April2012to December2013. The patients according the ration of1:1were randomly allocated to the Shu Mu points with bleeding therapy group and conventional acupuncture group, which included30patients in the Shu Mu points with bleeding therapy group and30patients in conventional acupuncture group. TCM diagnostic criteria referred to Higher education national planning materials seventh edition of "internal medicine "on "Insomnia" diagnostic criteria, and the western diagnostic criteria issued referred to "Chinese medicine clinical research guidelines ’China Classification and diagnostic criteria for mental disorders,"4th Edition (CCMD-4) diagnostic criteria for insomnia. The syndrome differentiation of TCM referred to the higher education national planning materials,"internal medicine" Insomnia dialectical type. Treatment for a total of two courses, a course is two weeks, three times a week. The curative evaluation standard mainly were according to Insomnia of "TCMsyndrome score standards" promulgated by the State Administration of Traditional Chinese Medicine in2002, the Pittsburgh Sleep Quality Index (PSQI), Athens Insomnia Scale (AIS).ResultsBefore the treatment, the Shu Mu acpionts with bleeding therapy and conventional acupuncture groups data analysis in sex, age and duration were not statistically significant (P>0.05), it showed that the clinical baseline data in two groups were comparable.The comparison of curative effects:Two groups by Wilcoxon rank sum test, z=0.118, P=0.042, was statistically significant (P<0.05), in which the number of Shu Mu acpionts with bleeding therapy group number cured43.3%, effective number accounted for26.7%,23.3%of the number of valid, invalid number of6.7%, the total effective rate of93.3%. The number of the conventional acupuncture group number accounted for30.0%of the cure, effective number of16.7%,23.3%of the number of valid, invalid persons accounted for30.0%, total effective rate was70.0%. It demonstrated that compared to the conventional acupuncture group, the Shu Mu acpionts with bleeding therapy was better therapy for treating insomnia of liver fire disturbing heart syndrome.The TCM syndrome score:After treatment, the two groups score of TCM syndrome score by t test, with statistically significant (P<0.05). It suggested that compared to conventional acupuncture group, the Shu Mu acpionts with bleeding therapy was more effective in improving the patients with insomnia, irritability, dizziness, dry mouth, mouth pain, chest fullness symptoms.In the Pittsburgh Sleep Quality Index (PSQI) score:After treatment, the Pittsburgh Sleep Quality Index (PSQI) score by t test were statistically significant (P<0.05). It suggested that the Shu Mu acpionts with bleeding therapy and the conventional acupuncture can improve sleep quality, sleep latency, sleep efficiency, obstacles, and so on, both treatments in improving the Pittsburgh Sleep Quality Index (PSQI) score was not significantly different terms.In Athens Insomnia Scale (AIS) score:the Shu Mu acpionts with bleeding therapy group before and after treatment Athens (AIS) score by t test, the difference was significant (P<0.01). Conventional acupuncture group before and after treatment Athens Insomnia Scale (AIS) score difference was statistically significant (P<0.05). It suggested that the two therapies both can improve sleep time, wake up at night, daytime aspects of emotional, physical function and daytime sleepiness, etc.ConclusionShun Mu points acupuncture, bleeding therapy as a treatment for insomnia traditional methods, used for many years, played a big role. In the clinical studies, mainly for insomnia, heart disease location characteristics, the choice of heart-Shu and Juque main points, and combined with the corresponding syndrome of insomnia with liver fire disturbing heart, we also used liver-Shu and Qimen, so as to achieve yin and yang, uneasiness of mind, so that patients with insomnia body recovery,"yin Pingyang secret’health status; Apex bloodletting to relieve insomnia patients with mental stress, increased sleepiness degree so as to achieve the purpose of treatment of insomnia. This research intends to observe the clinical efficacy of the Shu Mu points with bleeding therapy and the conventional acupuncture therapy, to find their own characteristics and scope to explore better treatment of the disease non-surgical therapy. In the clinical studies, mainly for insomnia, heart disease location characteristics, the choice of heart-Shu and Juque main points, and combined with the corresponding syndrome of insomnia with liver fire disturbing heart, we also used liver-Shu and Qimen, so as to achieve yin and yang, uneasiness of mind, so that patients with insomnia body recovery,"yin Pingyang secret’ health status; Apex bloodletting to relieve insomnia patients with mental stress, increased sleepiness degree so as to achieve the purpose of treatment of insomnia. This research intends to observe the cl inical efficacy of the Shu Mu points with bleeding therapy and the conventional acupuncture therapy, to find their own characteristics and scope to explore better treatment of the disease non-surgical therapy.The results show that both therapies have certain curative effects for insomnia with liver fire disturbing heart syndrome, can improve sleep quality in patients with insomnia, sleep time, sleep efficiency during the day emotional, physical function and so on. After treatment, the overall efficacy and comparative aspects of TCM symptom score, the Shu Mu points with bleeding therapy compared with conventional acupuncture, was better efftive. The Shu Mu points with bleeding therapy is a treatment for insomnia with liver fire disturbing heart syndrome effective, economical, convenient, non-toxic side effects of Chinese medicine.
