Objective:Analysis the clinical curative effect of the treatment of soft tissue d efects by using island flap combined with VSD(vacuum sealing drainage, VS D).Methods:Choose54cases whose soft tissue defects are more than6cm×8cm in113cases of soft tissue defect in lower leg and ankle that were collected fro m Februrary2011to February2012.54cases were randomly assigned,Simple f lap transfer as the traditional group(n=27) and flap transfer with VSD as VS D group(n=27),and treated with the island flaps technique combining with VS D. Flap type:14cases of the posterior tibial artery perforator pedicled island f lap,13cases of Fascia island flap in the traditional group;19cases of the post erior tibial artery perforator pedicled island flap,8cases of Fascia island flap i n the VSD group.The minimum size defect in the VSD group is6cmx8crri,Th e maximum size of soft tissue is10cm×12cm;the minimum size of soft tissu e defect in traditional group is7cm×8cm,the maximum size is12cm×12cm.A O classification of soft tissue defect of the VSD group in the lower leg is10: IO3in5cases,IO4in9cases.AO classification of soft tissue defect in the ankle is10:1O3in5cases,IO4in8cases. AO classification of soft tissue defect of the traditional group in the lower leg is10:IO3in6cases,IO4in8cases. AO classification of soft tissue defect in the ankle is10:1O3in6cases,IO4in7cases.Result:54patients were followed up for12to18months.VSD group hosp ital stay time is an average of7.6days, wound healing time was an averag e of15.6days, dressing change times of the wound was3.2times.Tradition al group hospital stay time was an average of13.3days;wound healing tim e was an average of37days, dressing change times of the wound was10.1times. Follow-up one month:VSD group of soft tissue wounds are healed a s IC type1(27/27).Traditional group19cases of soft tissue wound heal ed as the IC type1(19/27). Follow-up of6months:VSD group flaps we re healing well, the traditional group of26patients with wound healing as IC type1(26/27). Follow-up of1year:flaps of VSD group healed well, one case of traditional group were treated with skin grafting for soft tissu e defects, six months after the wound healing as IC type1.Conclusion:Application of VSD technology in the skin flap, flap transfer b y reducing the rate of postoperative infection and improve the survival rate of skin flap, reduce hospitalization time and wound healing time, and by reduci ng the staff changing frequency, can reduce the medical staff and the pain tha t patients were suffered in dressing change in the clinical work.
