Objective:Comparative analysis of the clinical efficacy of ulnar shortening surgery in the Madelung’s deformityMethods:Back in2012March to2013September,30patients with Madelung’s deformity in our hospital for treatment, and they all underwent operation treatment,25female,5male, age40-55year old, average47.5years old,16cases of the right wrist, left wrist in14cases, operation were performed by ulnar shortening osteotomy.Typing of Zheng Jun et al.:Type Ⅰ:8cases,16cases of type Ⅱ,6cases of type Ⅲ. The follow-up time was9months~18months, Pain was evaluated by pain score system of Jiranek.The wrist joint function was evaluated by the wrist joint function scoring system of Hong Yunfei, using the Gangopadyay and Packer satisfaction score system to rate the satisfaction after operation.Results:All the incisions healed well, Within8months of bone healing. The hospitalization time:5-11days, the average length of8D.①Pain scores:type Ⅰ preoperative, postoperative1,3,6,12,18months scores were:6.3,23.7,26.9,27.3,27.6,29.2. Type Ⅱ preoperative, postoperative1,3,6,12,18months scores were:6,19.2,22.6,26.3,27.5,28.4. Type Ⅲ preoperative, postoperative1,3,6,12,18months scores were:5.1,16.8,20.8,23.7,24.6,26.9.②The postoperative functional score:The excellent and good rate of type Ⅰ after1,3,6,12,18months of functions:87.5%,87.5%,100%,100%,100%, the total effective rate was100%. The excellent and good rate of type Ⅱ after1,3,6,12,18months of functions:81.3%,81.3%,93.8%,93.8%,100%, the total effective rate was100%. The excellent and good rate of type Ⅲ after1,3,6,12,18months of functions was66.7%,66.7%,66.7%,83.3%,83.3%, the total effective rate was83.3%.(3)Patient satisfaction scores:Type Ⅰ after1,3,6,12,18months satisfaction scores:8.9,9.2,9.4,9.6,9.8. Type Ⅱ after1,3,6,12,18months satisfaction scores:8.1,8.4,8.6,8.8,9. Type Ⅲ after1,3,6,12,18months satisfaction scores were:5.8,6.1,6.6,6.7,7.1. Application of SPSS17.0software for statistical analysis and t test, The postoperative1,6,18months compared with preoperative pain score were significantly increased (P<0.01). With the extension of time after operation, pain score more and more high, between each postoperative time were statistically significant (P<0.05). Type Ⅰ Ⅱ Ⅲ postoperative function of the excellent and good rate increased significantly (P<0.01). The postoperative1,6,18months of satisfaction scores were compared between groups, Type Ⅰ satisfaction scores the highest, the second is type Ⅱ, type Ⅲ minimum. Conclusion:Ulnar shortening osteotomy for treatment of Madelung’s deformity, Type Ⅰ and type Ⅱ had a significant effect, Type Ⅲ although have some improvement, but the patient satisfaction is lower than the other two.
