BackgroundsWith the continuously developing of obstetric and neonatal intensive care technology, birth rate is higher and higher. Although a majority of low weight birth and that of very low birth weight infant survival rate increase. At the same time, those infants associated with brain injury are also going up, which may seriously affect children’s physical, mental development and quality of life. According to the relevant reports both at home and abroad, the incidence of cerebral palsy is in the first row in infants with brain injury. Finally, the sort of children suffers from sequelae of movement, memory, learning and behavior in all aspects. Patients with cerebral palsy are physically and socially disabled, which has become a serious global public health problem. What are the causes of the forming to refractory cerebral palsy? Then, what are the mechanisms? How to prevent and treat this kind of disease? To this end, the medical profession has made unremitting efforts. At present, there is not a good method to detect the pathogenesis of cerebral pals y, the diagnosis, treatment and better prevention. Diffusion tensor imaging (DTI) is a noninvasive and new inspection method. It can display the nerve fibers of distribution, walking and deletion. In addition, which is sensitive to provide tiny pathological informa tion about the change of disease.ObjectiveThis study was designed to use magnetic resonance diffusion tensor imaging to evaluate changes of white matter fiber with cerebral palsy.It is direct to clinical diagnosis and pron gosis..MethodsThe17children suffered from cerebral palsy and6children age-matched normal control were recruited. At first, all children were examined by MRI and DTI, we can get FA maps and color coded FA maps. The anterior limb of the internal capsule, genu of internal capsule, the posterior limb of the internal capsule, corpus callosum, corticospinal tract, genu splenium of corpus callosum, cingulum bundle set a region of interest (ROI). FA in ROI was analysed by unpaired t test. All the children in this experiment were divided into normal, mild abnormal, moderate abnormal, severe abnormal by PVL. All different groups were compared with single factor analysis of variance. SPSS17.0software was applied for statistical treatment. P<0.05was considered statistically significant.ResultThe experimental group compared with the control group found the group of children with bilateral corticospinal tract, bilateral anterior limb of the internal capsule (ICAL)(left0.507±0.054vs.0.676±0.054);(right0.524+0.062vs.0.649±0.048), bilateral posterior limb of internal capsule (ICPL)(left0.561±0.056vs.0.690±0.053);(right0.543±0.057vs.0.702±0.053), genu of corpus callosum (GCC)(left0.613±0.068vs.0.732±0.061), corticospinal tract (CST)(left0.551±0.046vs.0.606±0.073);(right0.583±0.039vs.0.632±0.034),anterior(CO)(left0.315±0.091vs.0.440±0.055);(right0.345±0.098vs.0.416±0.057),bilateral cingulum bundle FA values significantly decreased, the differ ence was st atistically significant (t=3.176±4.884,P<0.05), found that all the FA values of fiber bund le decreased. The comparison between groups showed that the FA values of bilateral corti cospinal tract, bilateral anterior limb of the internal ca psule, bilateral posterior limb of in ternal capsule, corpus callosum, double genu of corpus callosum, bilateral cingulum bundl e were statistically significant different (F=5.774-34.245, P<0.05). According to2cases of children with DTI before and after treatment in contrast, color image showed improvem ent obviously after treatment, and the FA value significantly increased.ConclusionDTI scan can display white matter fiber damage, interrupt and abnormal.From the mi cro cosmic reaction changes in white matter tracts of the brain, it is the important value to the diagnosis of cerebral palsy.It can be used to guide to treatment for brain damage and ce reb ral palsy, comparison and efficacy.
