Objective: Combined with the research progress of Surgery First Approach, the aim ofthis thesis was to determine Surgery FirstApproach’s strengths and weaknesses ontreatment outcomes in skeletal Class ē malocclusions and to provide references byevaluating the therapy’s effects.Methods: Ten treated patients (5traditional combination of orthodontics andorthognathic surgery cases and5Surgery First Approach cases) were collected anddigitized from October2010to June2012in Dalian Stomatological Hospital. Theaverage pre-treatment age was24.76years (4males and6females). The procedure oftraditional combination of orthodontics and orthognathic surgery was pre-surgicalorthodontic treatment, orthognathic surgery and post-surgical orthodontic treatment.Whereas the Surgery First Approach required orthognathic surgery before orthodontictreatment’s decompensation and alignment. Lateral cephalometric films taken beforeand after treatment were analyzed. The effects were compared between the two groups.Results: The data comparison suggested that both groups’skeletal Class ē profile werecorrected and soft tissue profiles improved, but the group with traditional therapygained more thorough results (Items including SNB, L1-MP and shift distance ofmandible during surgery). There were significant differences in the period of treatmentbetween the two groups, displaying that group SFA had shorter duration of treatment.Conclusion: With strict implementation of indication and close cooperation betweenorthognathist and orthodontist, Surgery First Approach could be proposed to shorten theentire treatment period, to improve facial aesthetics from the beginning of treatment andto take advantage of the regional accelerated phenomenon for orthodontic toothmovement.
