Objective:Since elderly people get osteoporosis and other systematic chronic disease,hip muscles become vestigial and they lag in response as well, they can’t counteract theharmful stress of the hip effectively. So there is a high risk for elderly people getting hipfractures. Although re-fractures after the operation is not high, the rate of elderlypeople getting fractures is increasing in recent years, the absolute number of people sufferingfrom re-fractures keeps growing every year. Our research is aimed at analyzing the correlationbetween the incidence of offside re-fractures after the operation and some factors such asosteoporosis, summing up the clinical features of re-fractures after hip fracture operation,improving the perceptions of the risk factor of hip re-fractures after operation in order toreduce the hip re-fracture incidence.Methods:We selected562hip fracture cases in the bone surgery of Panjin SecondPeople Hospital from December2007to December2012(248transcervical fracture,314intertrochanter fracture of femur), from which we recorded the number of people with offsidere-fracture, the type of the fracture, BMD, age and gender, the reason of injury, thecorrelation among hip re-fracture and osteoporosis and other risk factors in detail, and studythe relativity through statistical analysis.Results: We collected562patients with hip fractures,31of which have offside fractures.The incidence of offside re-fractures is5.5%. Only136cases tested BMD in562patients. wearranged and analyzed the materials, and the results are as follows.①BMD is the mostimportant index to diagnose osteoporosis and evaluate the risk of hip fracture, the risk of hip fracture can be reliably forecasted through hip BMD test. These cases can be divided into twogroups:one side hip fracture and two side hip re-fracture. Comparing between the two groupsof BMD shows differences are significant (P<0.05).②Comparing between different typesof fractures to BMD shows the differences are significant (P<0.05). There is no significantdifference between the incidences of different types of fracture occurrence and offside hipre-fractures.(P>0.05).③The offside hip re-fracture incidences after hip fracture operationhas no statistical difference in gender (P>0.05).④Comparing different ages, the risk ofre-fractures increases with the age. The comparison between the data of the two groups issignificant (P <0.05)⑤For some basic level hospital without dual-energy X-rayabsorptiometry(DEXA), Singh index is a good way to know the osteoporosis level ofproximal thigh bone.Conclusion:1.Osteoporosis is the main risk factor of offside re-fracture after hipfracture operation.2. The incidence of hip re-fracture has no connection with fracture type,gender and injury reason. It increases with age.3.BMD is the most important index to diagnose osteoporosis and evaluatethe risk of hip fracture, the risk of osteoporosis and hip fracture can be reliably forecastedthrough hip BMD test, but BMD also has limitations.4.The Singh index has guiding significance for the diagnosis ofosteoporosis.
