ObjectiveWith pathological diagnosis as the gold standard, by magnetic resonanceimaging (MRI) for the diagnostic accordance rate of common benign ovariancystic lesion, to explore the application value of routine MRI scans andfunctional imaging studies in ovarian benign cystic lesion to provide reliablediagnostic basis for clinical.Materials and MethodsCollecting suspected ovarian cystic lesion in138cases between September2011and February2011in the hospital affiliated to taishan medical universityand ruijin hospital, all of them were performed routine MRI scan,56of whomunderwent DWI scans, and used the3D-MIP and Functiontool on DWI imagepost-processing to measure the ADC values of different pathological changesand calculate statistical analysis;45of whom underwent enhanced MRIscanning. Statistical analysis was calculated according to the reinforcementcharacteristics of different pathological change.Results1. In138cases,39cases of endometriosis cyst,34cases of ovarian cyst,25cases of ovarian mature teratoma,20cases of ovarian mucinous cystadenomaand14cases of ovarian serous cystadenoma were confirmed by pathology,6cases of other pathological changes. With pathologic results as the gold standard,the diagnostic accordance rate of ovarian benign cystic lesion was91.30%, themisdiagnosis rate was8.70%. The sensitivity, specificity, positive predictivevalue and negative predictive value in detecting endometriosis cysts were97.43%,97.98%,95%,98.98%respectively by MRI;The sensitivity, specificity,positive predictive value and negative predictive value in detecting ovariancysts were94.12%,95.19%,86.49%,98.02%respectively by MRI; The sensitivity, specificity, positive predictive value and negative predictive value in detectingmature ovarian teratomas were100%,99.12%,96.15%,100%respectively byMRI; The sensitivity, specificity, positive predictive value and negativepredictive value in detecting ovarian cystadenomas were91.18%,96.15%,88.57%,97.09%respectively by MRI.2. Analyzing enhanced scan features of5cases of endometriosis cyst,15cases of ovarian cyst and25cases of cystadenoma, endometriosis cysts wereclassified in the cases of ovarian cysts. Ovarian cysts were slightly enhanced in5cases,15cases had no obvious enhancement, cystadenomas hadmild-to-moderate enhancement in19cases,6cases had no obvious enhancement.By chi-square analysis, χ2=11.61, p﹤0.01, between the two was statisticallysignificant. That can through different reinforcement characteristics of ovariancyst and cystadenoma make a differential diagnosis.3. B value is equal to600,1000s/mm2, the ADC values of five kinds ofovarian pathological changes were measured respectively, and the ADC valuesof ovarian endometriosis cysts, ovarian cysts, mature teratomas, serouscystadenoma and mucinous cystadenoma were pairwise compared respectively.By multi-factor analysis of variance, except between serous cystadenoma andovarian cyst had no statistical significance(p=0.075>0.05), the otherpathological changes were obvious statistical significance(p<0.05).Conclusion1. The sensitivity, specificity and accuracy in detecting common benignovarian cystic lesion by MRI are very high.2. MRI-STIR can distinguish bleeding from fat, and diagnose teratoma,chocolate cyst and hemorrhage cyst with specificity.3. MRI enhancement scanning is good to identify ovarian cyst withcystadenoma.4. In five kinds of ovarian cystic lesion, with the exception of the ADCvalues of ovarian cysts and serous cystadenomas have no statistical significance,the rest are obvious statistical significance between the two, that manifests DWIand ADC values can identify ovarian cystic lesion.
