Objective: The author analyzed the characteristics of the its lipid metabolism bymeasuring the blood lipid level of hormone receptor negative breast cancer patients. Wealso discussed the correlation between dyslipidemia and distant metastasis of hormonereceptor negative breast cancer patients. Then we will analyze whether or not dyslipidemiais an independent factor for distant metastases. Dynamic observation of distant metastasisshowed that the change trend of the indicators in the level of blood lipids of hormonereceptor negative breast cancer patients before and atfer treatment, it helps us to analyze therelationship between blood lipids level and clinical curative effect.Methods: We collected clinical data of154breast cancer patients treated in xxxhospital rfom April2012to November2013,all patients had a definitive diagnosis ofhormone receptor negative breast cancer. Selected patients were divided into two groups,one group is breast cancer patients in local stage,the other group is distant metastasis ofbreast cancer.1We collected fasting venous blood collection of the patients in the morning, andanalyzed the level of serum total cholesterol, triglycerides, high-density lipoproteincholesterol (hdl-c) and low density lipoprotein. For patients with distant metastasis ofbreast cancer, we gave chemotherapy for2cycles according to the patient’s own situation aswell the NCCN guidelines,14days later, we obtained the determination of blood lipidindicators. Atfer2cycles of chemotherapy, we used imaging examination,adoptingRECIST criteria evaluation of curative effect, then divided them into three groups,partialresponse group (PR), stable condition (SD),disease progression group (PD). 2We analyzed the relationship between baseline lipid levels and distant metastasisthrough statistical analysis of clinical data,then we choosed significant factors, andanalyzed whether they are independent risk factors for distant metastasis of breast cancer;Statistical analysis showed the change of distant metastasis breast cancer before and atferchemotherapy and discussed the trend of each index and its relationship with the clinicalcurative effect.3We used SPSS21.0software for data analysis,single factor chi-square test waschoosed to compare the two groups; binary logistic regression analysis was choosed toanalyze risk factors for distant metastases; nonparametric test was choosed to analyze therelationship between blood lipid indicators change before and atfer chemotherapy andcurative effect. If P<0.05, it is considered that the two group is significantly different.Results:1The results through the single factor chi-square showed that factorsinfluenced on distant metastasis are: tumor size, regional lymph node metastasis andhistological grading,high blood TC level, high TG level,and high LDL-C hematicdisease, the difference was statistically significant {P <0.05); And there are no significantdifferences between distant metastasis and not distant metastasis in age,menopausal status,pathological type, albumin and low HDL-C concentration {P>0.05);2The results through the binary logistics regression analysis showed that theindependent risk factors for distant metastases are tumor size (OR=1.563),regional lymphnode metastasis (OR=1,983),high TC hematic disease (OR=1.502), high TG level (OR=1.877),histological grade and high LDL-C disease are not independent risk factors fordistant metastases;3We evaluated the curative effects of breast cancer distant metastasis patients atfer2cycles’chemotherapy, including20cases in PR group,42cases in SD group,21cases in PDgroup. The results through the nonparametric test showed that:PR group: TG level was (1.78+0.27) mmol/L,and it decreased atfer chemotherapy,the difference was statistically significant {P <0.05); TC level was (5.28+0.70)mmol/L,and it decreased atfer chemotherapy, the difference was no statistically significant {P>0.05); LDL-C level was (3.18+0.83)mmol/L,and it decreased atfer chemotherapy, thedifference was no statistically significant {P>0.05); HDL-C level was (1.25+0.27) mmol/L,it increased atfer the treatment, there was no statistically significant difference {P>0.05);SD group: TC level was (5.51+1.06) mmol/L, HDL-C level was (1.26+0.28)mmol/L, they both decreased after chemotherapy, and the difference had no statisticalsignificance (/>0.05); TG level was (1.84+1.11) mmol/L, LDL-C level was (3.48+0.72)mmol/L, they both increased after chemotherapy, the differences also had no statisticalsignificance {P>0.05);PD group: TC level was (5.48+1.15) mmol/L, TG level was (1.65+0.48) mmol/L,and LDL-C level was (3.45+0.99) mmol/L, and they all increased atfer chemotherapy,HDL-C level was (1,16+0.25) mmol/L, and it decreased after treatment, the differenceshad no statistical significance {P>0.05).Conclusion:1) The factors that influenced on distant metastasis of hormone receptornegative breast cancer are tumor size regional, lymph node metastasis,histological grading,high blood TC level, high TG level and high LDL-C level;2) The independent factors of hormone receptor negative breast cancer’s distantmetastases are tumor size (T),lymph node metastasis (N),high blood TC level and TGlevel;3) The lipid level of distant metastatic breast cancer patients is related to therapyefficacy, the TG level of PR group patients has a significant drop,that means anti-tumorcan effectively reduce the level of triglycerides;4) Dynamic monitoring blood lipid level can be used as an auxiliary index to diagnosedistant metastasis of hormone receptor negative breast cancer and evaluate curative effect.
