Objective: the natural hirudin has anticoagulant effect, anti thrombosiseffect, anti platelet aggregation effect and on a variety of cell regulation, theorycan promote free flap to reduce, promoting blood flow of free skin flap, isconducive to the growth of angiogenesis, improve the random flap survival rate.Random flap survival and angiogenesis formation is closely related toangiogenesis, the more higher, the survival rate of the flap.Methods: the self-made natural hirudin gel, and evaluate its quality, usingimproved Franz type diffusion cell for percutaneous penetration test, testing thehirudin activity using the Markwardt method. And pre clinical toxicological testof natural hirudin gel self made. Wiser rats were selected, in the design of therandom pattern skin flap, the area is9cmx3cm flap were randomly divided into3groups, respectively with saline, natural hirudin blank gel, gel matrix.Postoperative general, histopathology observation, seventh days to count the flap survival rate.Results: in vitro experiment: MarKwardt detected the active components ofnatural hirudin gel group can pass through the skin, and the empty gel group wasnot detected in any activity; local administration of the gel on experimentalanimal skin is safe. Experiment in vivo: postoperative7d, natural hirudin groupgel skin flap necrosis length significantly longer than the saline group, blank gelmatrix group, with significant difference (P <0.05). Postoperative3D, capillarynatural hirudin gel group in the dermis and micro vein red cell stasisphenomenon compared with the saline group, blank gel matrix groupsignificantly reduced; postoperative7d, natural hirudin subcutaneous collagengel group and the proliferation of granulation tissue was obviously better thanthat of saline group, blank gel matrix group; postoperative7d, natural hirudingel group flap survival rate higher than those in the saline group, blank gelmatrix group, with significant difference (P <0.05). The results show thatnatural hirudin can penetrate the skin tissue.Conclusion: local application of hirudin hydrogel also has the effect ofimproving the skin flap blood stasis, accelerate angiogenesisSignificance: through the experiment research to understand the naturalhirudin gel preparation and injection preparation can promote arbitrarily reduceflap congestion, improve skin flap survival rate. Lay the foundation for future clinical application of natural hirudin gel preparation in the treatment of congested flap.
