Objective To investigate the clinic effectiveness of minimally invasivesurgery(MIS)assisted alteplase in treatment of HICH.Methods Patients(85cases)searched in our hospital between May2010and June2013divided into two periods,who suffered from HICH uppertentorium of cerebellum and treated with MIS assisted two kinds ofthrombolytic drugs,were retrospectively analyzed and divided into thealteplase group(34cases),urokinase group(51cases).Of the patients at thetime of admission and prior to starting treatment and at the conclusion ofthrombolysis were analyzed with hematoma volume,brain edemavolume,midline shift,NIHSS and GOS et al.Results The hematoma volume(T=13.77, P=0.000,T=19.40, P=0.000),brain edema volume(T=7.69, P=0.000,T=10.87, P=0.000) and NIHSS (T=8.6, P=0.000,T=11.99, P=0.000)decreased significantly in the two groups,the reductionin midline shift(T=8.7, P=0.000, T=10.32, P=0.000) was significantly thesame, there were statistically significant difference; Compared with theurokinase group,the percent change in hematomavolume(T=3.03, P=0.003),brain edema volume(T=2.27, P=0.03),thepercent reduction in midline shift(T=2.55, P=0.001),GOS(T=2.07, P=0.04) and the time pulling out the catheter(T=-2.4, P=0.02) in the alteplase group were significantdifferences,except for NIHSS(T=2.03, P=0.07) and duration ofhospitalization (T=-0.9,P=0.37).Conclusion MIS combined with thrombolysis in HICH seem to be asafe and effective therapy,and the efficacy of alteplase is better thanurokinase.
