ObjectiveTo investigate the protection of jiangya tablet series on traditional Chinese medicine against target organ damage in spontaneously hypertensive rats (SHR), primarily explore the mechanism of anti-vascular and cardiac remodeling for clinical application.MethodsSHR were assigned randomly into5groups,10per set:the positive control group, the blank control, the other three groups treated with different doses of jiangya tablet series (jiangya Ian,1v and huang). Wistar-kyoto (WKY)-rats were used as control group. After gavage for12weeks, by using noninvasive sphygmomanometer to measure last arterial blood pressure, left ventricular posterior wall thickness(LVPWT), interventricular septal thickness(IVST), and left ventricular end-systolic diameter(LVESD) were measured by ultrasound, the data were used to calculate the left ventricular fractional shortening(LVFS), left ventricular end-systolic volume(IVESV), left ventricular end-diastolic volume,IVEDV), ejection fraction(LVEF) and stroke volume(SV); Brain Natriuretic Peptide(BNP) in serum wered determined by the forward method; renin activity (PRA), angiotensin Ⅱ (Ang Ⅱ), aldosterone (ALD), thromboxane B2(TXB2) and6-keto-prostaglandin F1a (6-Keto-PGF1a)in plasma were measured by radioimmunoassay; weighed heart and left ventricular from rats to calculate cardiac index(CI)and left ventricular index(LVMI), observed pathological changes in left ventricular under light microscope stained with HE, using pathological image analysis system to determine cardiomyocytes transection diameter(CTD); thoracic aorta from rats were observed the large artery wall morphological changes under light microscope stained with HE, using pathological image analysis system to determine media thickness(MT)of vascular.Result1. Effect of Jiangyabao series on SHR echocardiographyCompared with the WKY group, the LVEDV, LVESV and SV in SHR group were decreased; though the LVST, LCPWT, LVFS and EF were increased, no statistically significant. The jiangyabao series can significantly reduce the FS and EF, but showed no significant effect on other echocardiography indicators.2. Impact on SHR plasma PRA, Ang Ⅱ, ALD and serum BNP’s in SHRCompared with the WKY group, the PRA activity and Ang Ⅱ content in plasma of SHR were decreased, the BNP levels were significantly higer and the ALD showed no significant change; jiangyabao Ian, huang and lv could reduce the PRA activity and Ang Ⅱ content in plasma, signigicantly elevated serum BNP levels, but no significant effect on ALD. The correlation coefficients of BNP, PRA, Ang Ⅱ and ALD were-0.94931,-0.70654and-0.24367.3. Impact on SHR plasma TXB2and6-Keto-PGF1aCompared with WKY group, the TXB2,6-Keto-PGF1a content and their ratio were no significant change; jiangyabao Ian, huang and lv could increase6-Keto-PGF1a, significangtly reducing TXB2/6-Keto-PGF1a, but had no significant effect on TXB2content.4. Pathological effects on SHR left ventricular morphologyCompared with WKY group, the CI and LVMI was significantly increased, left ventricular myocardial cell diameter was significantly increase, part of the cardiomyocytes hypertrophy under the microscope, myocardial fiber swelling degeneration, disorganized, broken and could see the infiltration of inflammatory cells. Jiangyabao series could significantly reduce cardiac index and left ventricular index, myocardial pathological changes had improved to varying degrees.5. Effect on SHR thoracic aortic pathology formsCompared with WKY group, the SHR’s aortic intima was thickening, and there was a significant difference (P<0.01), vascular smooth muscle ande elastic membrane disorder, the number of nucleus per unit area was significantly increased, and some cells appeared intimal injury and inflammatory cell infiltration outer membrane. Each jiangyabao group and enalapril group’s aorta intima was lower than SHR group, and the vascular smooth muscle and elastic membrane disorder had improved significantly.Conclusion1. The20-week-old SHRs have obvious pathological changes, such as vascular and cardiacremodeling.2. Jiangya tablet series have varying degrees prevention and reversal effect on vascular and cardiacremodeling in rats with hypertension. 3. Parts of the mechanism of Chinese medicine against hypertensive vascular and cardiac remodeling are related to the regulation of BNP, RAAS and the balance arachidonic acid metabolism.
