Objective(1) This study was to design stroke navigation chart of community health service to provide reference for community medical staff.(2) This study was to use patient navigation to provide health education for stroke and to verify the effects.(3) This study was to testify the effect of patient navigation.Methods102stroke patients in Qinling and Nanguan communitis were randomly divided into intervention group (patient navigation intervention) and control group (regular community intervention), the total intervention time is3months. The data was collected by stroke knowledge, attitude and behavior questionnaire and stroke rehabilitation care resources using knowledge, attitude and behavior questionnaire at baseline,1month and3months after the intervention. Two independent sample t-est, pair t-est, repetitive measure analysis of variance were used to analysis the data and to evaluate the effect of the patient navigation intervention.ResultsThe overall score as well as the the knowledge, attitude and behavior scores of the stroke knowledge, attitude and behavior questionnaire of the two groups were both improved (P<0.05) at1month,3months after intervention. The rising trend of intervention group score is more obvious than control group. The overall score as well as the three parts scores of the stroke knowledge, attitude and behavior questionnaire of intervention group were higher than control group (P<0.05) respectively at1month,3months after intervention.The overall score as well as the knowledge, attitude and behavior scores of the knowledge, attitude and behavior questionnaire of stroke rehabilitation care resources using of the two groups were improved (P<0.05) at1month,3months after intervention. The overall score as well as the three parts scores of the knowledge, attitude and behavior questionnaire of stroke rehabilitation care resources using of intervention group were higher than control group (P<0.05) respectively at1month,3months after intervention.ConclusionPatient navigation can effectively improve the health behavior and rehabilitation care resource utilization of stroke patients, and promote the community rehabilitation of stroke patients.
