Objective To investigate the Evaluation of MR perfusion weighted imaging in prognosis of young patients with MCA stenosis.Methods Patients under45years old with transient ischemic attack(TIA)(n=60) were selected from the Department of Neurology of the Affiliated Hospital of Jining Medical College from the January2011to June2013. All cases with unilateral middle cerebral artery (MCA) stenosis or occlusion was diagnosed by DSA. A control group of60healthy subjects was randomly selected from the medical center of the same hospital, matched for age and sex. Degree of stenosis was graded as mild (0-29%), moderate (30-69%), severe or occlusion (>70%). PWI were done on the patients during interphase of TIA. All cases were followed-up from one to two years, and were be divided into infarction group and noninfarction group. The difference of PWI indexs between two groups were compared.Results.1.Twelve cases of mild stenosis were diagnosed. There was no significant difference in PWI indexes of the involved cerebral hemispheres between cerebral infarction group and noninfarction group.2.There were twenty-eight cases with moderate stenosis. The rMTT and rTTP of the involved cerebral hemispheres were significantly increased in cerebral infarction group compared with noninfarction group.3. Twenty cases with severe stenosis or occlusion were diagnosed. Compared to noninfarction group, the rCBV was significantly reduced and the rMTT and rTTP of the involved cerebral hemispheres were significantly increased in the infarction group.Coneluosion1.There is significant value in evaluation of PWI in prognosis of patients with moderate MCA stenosis and above.2.PWI is fast and easy to be operated, and also easy to be accepted by patients. It is suggested that the status of blood supply for the cerebral tissues may be exactly assessed by MR PWI, which is an sensitive index for evaluation of prognosis patients with MCA stenosis.
