Purpose:To test expression of peripheral blood and tissue of micro RNA155(miR-155) and micro RNA196a (miR-196a) in pancreatic cancer, and the joint of three tumor markers CA199, CA242and CEA, explore the role and significance of the miRNA and tumor markers in the diagnosis and treatment of pancreatic cancer. Methods:The60cases of pancreatic cancer patients were collected by clinical dignosed or by the image, surgery;20patients with chronic pancreatitis or pancreatic cancer lesions,10cases of normal healthy donors, the clinical data and peripheral venous blood;10cases of pancreatic lesion by surgery or endoscopic ultrasonography-guided fine-needle aspiration(EUS-FNA).Use real-time fluorescence quantitative polymerase chain (qRT-PCR) technology to detect miR-155, miR-196a expression in the above cases. At the same time the above cases of three tumor markers detection value of CA199, CA242and CEA had been collected.Results:1.The relative expression of plasma micRNAs The relative expression of plasma.miR-155, miR-196a in patients with pancreatic cancer is significantly higher than those with chronic pancreatitis and normal control group. The relative expression of miR-155in PCa group was6.11±7.89, CP groupl.68±2.95, Norm group0.4±1.6(bothP<0.01).The relative expression of miR-196a:PCa group8.52±9.44. CP group2.83±2.35and Norm group0.6±1.3(bothP<0.01). The relative expression of those two miRNAs in pancreatic cancer are higher than chronic pancreatitis and normal.2.The relationship between plasma miRNA levels and clinical characteristics in patients with pancreatic cancer In pancreatic cancer patients,the plasma miR-155, miR-196a level and clinical characteristics such as age, gender, tumor diameter, the relationship between miR-155have nothing to do with the above characteristics, miR-196a is only related with lymph node metastasis.3.The area under the ROC curve According to the principle of diagnostic screening test to draw the receive operating characteristic curve of plasma miR-155, miR-196a, the bigger of area under the curve, the greater the diagnostic efficiency. AUC-ROC of plasma miR-155, miR-196a in pancreatic cancer+normal controls respectively were0.601(95%CI:0.588-0.648),0.632(95%CI:0.603-0.664), pancreatic cancer+chronic pancreatitis ROC were0.644(95%CI:0.621-0.675),0.671(95%CI:0.663-0.687). In pancreatic cancer, chronic pancreatitis and normal control all3groups ROC were0.758(95%CI:0.711-0.802),0.772(95%CI:0.734-0.822). Through the statistical number,we can get that miR-155,miR-196a has diagnostic value. Those of area under the ROC curve in0.70-0.90, shows that these two kinds of micrornas in the diagnosis of pancreatic cancer accuracy as the medium.4.Sensitivity and specificity of plasma miR-155in patients with pancreatic cancer were71.1%.63.3%; sensitivity and specificity of miR-196a were73.4%.65.8%;two kinds of sensitivity and specificity microRNAs were77.4%and70.6%. Three kinds of serum tumor markers CA199, CA242and CEA in the sensitivity and specificity were65.4%and60.7%. Results suggest the miRNA high sensitivity and specificity than three kinds of serum tumor markers CA199, CA242and CEA.5.Tissue in the quantity expression of miR-155and miR-196a were respectively:7.32±8.01,8.94±10.67. Plasma and tissue in the quantity expression of miR-155and miR-196a were no significant difference.Conclusion:QRT-polymerase chain reaction (PCR) detection of micrornas has high sensitivity and specificity. High expression of miR-155, miR-196a in pancreatic cancer, there are likely to be specific tumor markers in early diagnosis of pancreatic cancer. And miR-196a is associated with lymph node metastasis, which may indicate the progress of the pancreatic cancer. Effectiveness of miRNAs in the diagnosis of pancreatic cancer is better than that of three kinds of serum tumor markers CA199. CA242and CEA; for patients at risk for pancreatic cancer can joint detection miRNA expression and serum markers CA199, CA242, CEA to improve diagnostic efficiency of pancreatic cancer.EUS-FNA can become an important method for diagnosis of pancreatic diseased tissue, combined detection with the miRNA and pathology, which become the main minimally invasive examination to distinguish benign and malignant pancreatic diseases.
