Objective:To investigate the correlation between the expression level of costimulatory molecules B7-H3, B7-H4and prognosis value in patients with gastric cancer.Methods:A retrospective study was performed to investigate the clinical data of157patients with gastric cancer. Expression level of pathological indicators and B7-H3, B7-H4were detected by the method of immunohistochemical. The correlation between different expression level of B7-H3, B7-H4and clinical characteristics were tested by chi-square test. PFS and OS of different clinical and pathological characteristics were compared using Kaplan-Meier and Log-rank test; The Cox proportional hazards model was performed to estimate HR (Hazard Risk) with95%CI (confidence interval) of the linking strength between the different clinical characteristics, expressions of B7-H3and B7-H4and recurrence, death risks.Results:There was no obvious correlation between B7-H3, B7-H4expression levels and the different clinical characteristics. The results of univariate survival analysis showed that the older (>60years), staging late, larger tumors (>5cm), deeper infiltration (myometrial invasion), distant metastasis and costimulatory molecule B7-H4high expression were important factors which significantly reduced both progression-free survival (PFS) and OS (overall survival). Their P value are all less than0.05. The results of Cox model showed that the HR of recurrence and death of the patients with stage IV were6.20(95%CI:1.31-29.34),5.58(95%CI:1.27-24.58) respectively when compared to patients with stage I. The HR of recurrence and death of the patients whose tumor size>5cm were1.97(95%CI:1.19-3.25),1.95(95%CI:1.20-3.18) respectively compared to the patients whose tumor size was≤5cm. The HR of recurrence and death of the group of B7-H3low expression jointed with B7-H4high expression were1.92(95%CI:1.20-3.10), 1.93(95%CI:1.19-3.10) respectively compared to other classifications.Conclusion:B7-H3and B7-H4expression status can significantly affect the PFS and OS of gastric cancer patients. Combination B7-H3and B7-H4may improve the prediction quality of gastric cancer patients prognosis.
