| Objective:To investigate the current blood glucose compliance status of582patients in type2diabetes in endocrine outpatients,analyze glycosylated hemoglobinA1c (HbA1c)controls influencing factors.Ethods:Investigation of the selected condition of582cases with the2013May toDecember in endocrine outpatients with type2diabetes mellitus, collection of patientswith general information, laboratory results and treatment programs, using HbA1c<7%as the standard of blood glucose control, in order to understand the endocrine clinic inour hospital control situation of type2diabetes, according to gender, age, course ofdisease, degree of culture, regular monitoring of blood glucose, whether or not to acceptthe health education of diabetes, treatment options were grouped, and to explore theinfluencing factors of HbA1c standard.Result:1.We investigated in582patients with type2diabetes millitus,338males,244females, compliance rate of HbA1c was30.2%.2.According to the course of the disease, the patients were divided into threegroups, the course of <5years group’s (145people) level of HbA1c was7.1%±1.6%,HbA1c compliance rate was35.9%; the course of5-10years group’s (274people) levelof HbA1c was7.4%±1.2%, compliance rate of HbA1c is32.1%; the course of>10years group’s (163people) level of HbA1c was7.8%±1.7%, compliance rate ofHbA1c was22.1%. The comparison among groups was statistically significantdifference (p<0.05).3.According to the degree of culture, the patients were divided into two groups,receiving higher education group’s (139people) level of HbA1c was7%±1.1%,compliance rate of HbA1c was41%; those without higher education group’s (443people) level of HbA1c was7.3%±1.5%, compliance rate of HbA1c was26.9%. Therewere statistically significant differences between the two groups (p<0.05). 4.According to whether the law of self monitoring of blood glucose, the patientswere divided into two groups, regular self monitoring of blood glucose group’s (105people) level of HbA1c was6.9%±1.2%, compliance rate of HbA1c was59%;irregular self monitoring of blood glucose group’s (477people) level of HbA1c was7.6%±1.3%, compliance rate of HbA1c was23.9%. There were statistically significantdifferences between the two groups (p<0.05).5.According to whether or not to accept the health education,the patients weredivided into two groups, the group’s (236people) level of HbA1c was7.2%±1.4%whoaccepted diabetes health education, compliance rate of HbA1c was37.7%; the group’s(346people) level of HbA1c was7.5%±1.3%who didn’t accepted diabetes healtheducation, compliance rate of HbA1c was25.1%. There were statistically significantdifferences between the two groups (p<0.05).6.According to the treatment, the patients were divided into three groups, oralhypoglycemic drugs group, the simple application of insulin treatment group and insulincombined with oral drug therapy group. The group who was given Oral hypoglycemicdrugs had393people (67.5%), the duration was6±5.1years, the level of HbA1c was7.2%±1.6%, compliance rate of HbA1c was31.3%; the group who was given insulintreatment had77people(13.2%), the duration was7.6±6.1years, the level of HbA1cwas7.4%±1.5%, compliance rate of HbA1c was24.7%; the group who was giveninsulin combined with oral drug treatment had112people(19.2%), the duration was8.4±6.2years, the level of HbA1c was8.3%±1.4%, compliance rate of HbA1c was30.8%. The group who was given insulin combined with oral drug treatment had thelongest duration, the highest levels of HbA1c, compared with other groups, thedifference was statistically significant (p<0.05,*p<0.05,△p<0.05)..According to thetype of oral hypoglycemic drugs, the patients were divided into an oral hypoglycemicdrug group, two oral hypoglycemic drug group and three oral hypoglycemic druggroup. The group who was given three oral hypoglycemic drug treatment group had thelongest duration, the highest levels of HbA1c, the lowest HbA1c compliance rate. Therewere statistically significant differences between the three groups (p<0.05,*p<0.05, △p<0.05).Conclusion:1.The compliance rate of HbA1c was30.2%in Yantai Hospital Affiliated toBinzhou Medical University endocrine outpatients582patients in type2diabetes.2.The difference in the gender and the age was nothing with the compliance rate ofHbA1c. With the level of education improved,SMBG accepted regularly patients andthe health education accepted, the compliance rate of HbA1c would increase. Differenttreatment would lead to the difference of the compliance rate of HbA1c. The group whowas given three oral hypoglycemic drug treatment group had the lowest HbA1ccompliance rate. |
