Objective: Clara cell secretory protein (CC16) was applied on traumatic shock ratsto investigate the protective effect on reperfusion injury of the shock secondary lunginjury.The mechanism was studied preliminarily and investigated.Provid new ideas andnew mentality for the treatment of traumatic shock lung injury.Methods:Adult healthy male30SD rats (300-350)g,were randomly divided into3groups:Sham-operated group,controlgroup,CC16treatment group,each group has thesame number of animals and no significant difference.All animals were fasted overnightbefor operation,free drank water,sleep and rest,10%chloral hydrate(0.3ml/100g,intraperitoneal injection) under anesthesia,making a incision at about2mmaround the midline of the neck,20arteriovenous indwelling needle need to beacupunctured the right carotid vein,artery connecting multiple physiologicalmonitoring,and monitoring of artery blood,venous catheter for infusion,sham operationoperating at this point;the other two groups,in the condition of monitoring the bloodpressure,clamping the middle under1/3of the left femur with surgicalforceps,hemostasis,bandage up,when the blood pressure stable,make the ratsbloodletting5ml from the carotid artery quickly,to make sure the average arterypressure drop to35-40mmHg.After the blood pressure stable make the control group aerosol inhalation of0.9%saline for10minutes;After the blood pressure stable,w henthe physiological indicators stable,make the CC16treatment group aerosol inhalation of50ug/ml restructuring human clara cell secretory protein (rh-CCSP) for10minutes.After90min,making the two groups resuscitation respectively,using autologousblood and lactated ringer’s solution,twice the rehydration quantity of blood loss.TheSham-operated group were completed all the operation,but do not dispose anyadditional,no bloodletting and resuscitation;between the time of bloodletting and fluidresuscitation,the control group aerosol inhalation normal saline the same amount ofCC16treatment group,the most difference between the control group and the CC16treatment group is the difference in the substances for inhalation,the CC16treatmentgroup were aerosol inhalation restructuring human clara cell secretory protein (rh-CCSP)0.1μg/ml before resuscitation.At the same time,monitoring arterial blood gas analysisand lung wet/dry weight ratio changes and so on,at the same time also determinemalondialdehyde (MDA),myeloperoxidase(MPO) of lung tissue,in the meantime surveythe change of the lung tissue pathology.Results:1.arterial blood gas analysis.CC16treatment group the lung tissueoxygenation of rats are improved considerably,dramatically improve oxygenationindex,PH PaO2value increased significantly,BE value decreased (P<0.05),seen fromthe vaule that CC16treatment group and Sham-operated group have no statisticalsignificance (P>0.05).2.lung wet/dry weight.Compared with the control group,CC16treatment group lung wet/dry weight ratio decreases (P<0.05);CC16treatment groupand sham operation have no statistical significance (P>0.05).3.The myeloperoxidase(MPO)and malondialdehyde (MDA) content of lung tissue and serum.In the press oftraumatic shock lung damage,lung tissue have oxidative stress response,activation ofoxygen free radicals can be mediated adhesion reaction of white blood cells,thusaggravate the deterioration of lung tissue.Compared with the control group,CC16treatment group have a significant decreased of MDA,MPO in lung tissue andserum,CC16obviously alleviate the oxidative damage of lung tissue4.Lung tissue HEdyeing.The results showed that the group applicate of CC16can relief inflammatory reaction,the degree of pulmonary edema alleviated,alveolar hyperemia and hemorrhageare improved.Conclusion:1.rh-CCSP can effectively alleviate the lung injury of animals,improveits function and tissue oxygenation,decrease extravascular lung water and capillarypermeability index,its role of a dose effect relationship.2.rh-CCSP can effectivelyimprove the pulmonary tissue antioxidant enzymes activity,part of the mechanism of theprotective effect on animal acute lung injury is realized through theregulation ofendogenous antioxidant enzyme activity.The inflammatory reaction of inhaled rh-CCSPcan reduce thesecondary lung injury in a rat model of traumatic shock,effectivelyreference CC16has a protective effect on lung tissue.
