Objective: To collected and observe the clinical data of epithelial ovarian cancerpatients.Grouped the epithelial ovarian cancer patients by whether associated withendometriosis or not.Analysis of the characteristics and differentiations of the twogroups.Take for example,clinical presentation,histological subtypes, pathological stage,pathological grade,survival prognosis. Investigate whether there are some uniquefeatures about clinical, pathology, prognosis in the epithelial ovarian cancer withconcurrent endometriosis. To know whether the epithelial ovarian cancer patients withconcurrent endometriosis may used to be a factor to effect on the survival of epithelialpatients.Methods: Take the epithelial ovarian cancer patients whom diagnosis and surgeryin Dalian Maternity Hospital during2002.01.01-2009.01.01.In totals of233cases werecollected in our study.Take a retrospective study with them.Group the epithelialovarian cancer patients by whether with concurrent endometriosis or not.But excludethe case which may comes from the malignant transformation ofendometriosis.Observed the feature about their clinical and pathology.Then takepostoperative follow-up on them. Analysis whether there were some differences in thethe two groups and whether the differences were statistical significance bySPSS17.0.Calculate the5-year survival statistics of the two groups and draw thesurvival curves.After take the epithelial patients in two parts,early stage group and latestage group.Then use the same way to compared their survival respectively.Results: There were31epithelial ovarian cancer with concurrent endometriosispatients in the collected233epithelial ovarian cancer patients.The mean age of them was45.74.They were4years younger then the epithelial ovarian cancer patientswithout concurrent endometriosis.Most of the patients’clinical presentation was of pelvic pain and pelvic mass.Mostof their histological subtype was clear cell and endometrioid ovarian cancers.Histological subtype is a maximum proportion of clear cell carcinoma of the ovaryaccounts for38.7%. Followed by endometrioid adenocarcinoma of the ovaryaccounted for29.0%.For the202cases of epithelial ovarian cancer patients who without concurrentendometriosis the majority of their histological subtypes was serous adenocarcinoma ofthe ovary,which accounting for45.5%. And the distribution of histological subtypes forthe two groups exists differences.There are more early stage in the epithelial ovariancancer patients with concurrent endometriosis than the other.The percent of the earlystage in the frist group is74.2%.Especially FIGOI account for about61%.In contrastto this, there is most late stage in the epithelial ovarian cancer withoutendometriosis.Especially for FIGOIII.There are differences between the two groups regard to histological stage.Alsofor the pathologic differentiation of the two groups. There are statistical differencesbetween the two groups.The numbers of the low-grade case in the group for epithelialovarian cancer patients with concurrent endometriosis were letter and the otherones.The proportion is25.8%and71.8%respectively.Comparing the two groups about the patients’prognosis.The epithelial ovariancancer patients with concurrent endometriosis had a5-year survival rate of71.0%andthe epithelial ovarian cancer patients without endometriosis had5-year survival rateof only51.98%.The differences between them is statistically significance.Conclusion: The probability of epithelial ovarian cancer patients with concurrentendometriosis was high. Their clinical presentation often more obvious in the earlystage.Most of their histological subtype was clear cell carcinoma and endometrioidadenocarcinoma of the ovary. There are more patients in early stage especially FIGOIin them.The differences of their pathologic differentiation and the survival prognosis are have statistically significance.The epithelial ovarian cancer patients with concurrentendometriosis was better.The advantage about the survival prognosis in the epithelialovarian cancer with concurrent endometriosis may comes from the earlier stage and thelower grade which rise from the more obvious clinic symptoms.But after we grouppatients by early stage group and late stage group we found that patients in the latestage group who concurrent endometriosis also have a better survival.It shows that thefactor of whether an epithelial ovarian cancer with concurrent endometriosis may has agood effect on the survival prognosis of the epithelial ovarian cancer patients.But in ourstudy the numbers of late grade ovarian patients is less and there is still a lot to study inthe genetic level about them.So whether we can use endometriosis to be a factor toaffect the prognosis of epithelial ovarian cancer patients needs a larger sample ofclinical research to prove.
