| Objective: To investigate the clinical significance of radiofrequencythermal ablation (RFA) combined with transarterial chemoembolization (TACE) inhepatic metastasis of gastric cancer. Methods: This study enrolled23gastric cancerpatients with hepatic metastasis of less than4lesions and diameter of no more than4cm.All patients undertook radical gastrectomy with D2lymphadenectomy, intraoperativeRFA were undertaken under the ultrasonic guidance. Among the23patients,15receivedTACE preoperatively. Postoperative chemotherapy was performed in the majority ofpatients by fluorouracil and oxaliplatin based regimens. Results:This series of patientshad no severe complications. The recurrent free and overall survival was366days and505days,5patients gained long term survival without recurrence. Multivariate analysisindicated that N staging, distribution of metastasis and TACE therapy were independentprognosis factors. Stratification analysis indicated that preoperative TACE wassignificant in multi-lobes metastasis cases. Conclusion: Sole or manipulus hepatic metastasis of stomach cancer could be effectively treated by multidisciplinary therapysuch as radical gastrectomy, RFA, TACE and postoperative chemotherapy. In cautiouslyselected patients this strategy might be potentially curative. |
