1Objective:To investigate the value of application use acoustic radiation force impulseimaging techniques(Acoustic Radiation Force Impulse, ARFI)in the diagnosis of benignand malignant thyroid nodules.2Method: We selected a total of80cases of patients in our hospital since September2012to December2013with the diagnosis and treatment of thyroid nodules.Randomly selected30cases of normal volunteers to participate in research at the same time, as the controlgroup. We detect all the lesionsvia VTI and VTQ.Then get the VTI of different thyroiddisease and VTQ of different thyroid disease and surrounding tissues.Using thepostoperative pathology as a standard to investigate the value of application use acousticradiation force impulse imaging techniques(Acoustic Radiation Force Impulse, ARFI)inthe diagnosis of different thyroid diseases. At last, to analyze the differences in patientsatisfaction.3Results: There were53benign nodules and27malignant nodules. In benign group,30.2%nodules showed hyperechoic on VTI elastic graph, indicating that the tumor is soft texture;22.6%nodules showed hypoechoic on VTI elastic graph, indicating that the tumor is hardtexture;47.2%nodules showed isoechoic on VTI elastic graph, indicating that the tumorhad the similar texture compared with the surrounding tissue. In malignant group,11.1%nodules showed hyperechoic on VTI elastic graph;51.9%nodules showed hypoechoic onVTI elastic graph;37%nodules showed isoechoic on VTI elastic graph. Suggesting thatmalignant group was given priority to with a hard texture.. The VTQ results showed thatthe shear wave velocity of patients in benign thyroid was lower than that in the malignantgroup with a significant difference (P<0.05).Compared with the normal group, the shearwave velocity of patients in benign thyroid was higher than the normal group with asignificant difference(P <0.05). The shear wave velocity of goiter patients was higher thanthat of the thyroid adenoma and the thyroid hyperplasia with a significant difference(P <0.05). There was no significant difference in shear wave velocity between thethyroid adenoma and the thyroid hyperplasia(P>0.05).In malignant tgroup,The shearwave velocity of the papillary carcinoma was higher than that of the follicular carcinoma and the medullary carcinoma with a significant difference(P <0.05). The shear wavevelocity of follicular carcinoma was higher than that of the medullary carcinoma with asignificant differenc(eP <0.05).The satisfaction(97.5%)of ARFI detection was higher thanthat of the pathology(63.3%)group with a significant difference(P <0.05).4Conclusion: ARFI provides a new idea for the clinical diagnosis of thyroid dieases, complementing the traditional diagnostic methods, It has a certain reference value inidentification of the diagnosis of benign and malignant thyroid nodules.
