ObjectiveTo explore the feasibility,security and advantage of laparoscopicradio frequency ablation(LRFA) for the small liver cancer of the specialposition and to evaluate the curative effect.Methods90lesions in73patients with different degree of liver cirrhosispatients with hepatocellular carcinoma were checked by conventionalultrasound (US),abdominal computed tomography(CT),abdominalMRI,intraoperative LUS. The study analysis the situations of theinspection results and selects needle placement scheme according totumor size,records of the treatment process,the ablation needle number,cloth time and so on. The change value of the liver function,AFP,adverse reaction,the vascular flow to tumor nodules are compared beforeand after PRFA and LPRA treatments in order to observe the clinicalcurative effect and evaluate the feasibility,security and advantage oflaparoscopic radio frequency ablation(LRFA) for the small liver cancerof the special position and to evaluate the curative effect. Results1. The LRFA and PRFA therapies were successfully performedwithout serious complications in all patients. The changes of liverfunction and adverse reactions were no significant differences(P>0.05).Tumor necrosis,AFP value and blood supply of the tumor situations weresignificantly changed after treatment and the differences have statisticalsignificance(P<0.05). Also, LRFA has its certain advantages in thecomplete necrosis rate of tumor, the blood supply of the tumor gradings.2. The rate of LUS detecting lesions with diameter less than1.5cm,diagnosis and localization is better than that of US. There was significantdifference between two groups (P<0.05).Conclusions1. Laparoscopic radiofrequency ablation (LRFA) treatment of smallhepatocellular carcinoma in the special position is an effective and safemethods.Its short-term effective was accurated and satisfied.2. Laparoscopic ultrasound (LUS) guidance can improve theaccuracy of diagnosis and localization which become an importanttechnology to assist and complete the operation smoothly.
