objective:To compare the impact of different caloric parenteral nutrition on postoperativepatients Undergone radical resection for carcinoma of stomach on serum albumin(ALB), blood glucose, serum alanine aminotransferase (ALT) and aspartateaminotransferase (AST), Complication,body weight change effect, postoperativehospital stay and cost of hospitalization.Materials and methods:This is a case-control study. Statistics the patients of Jiangxi People’s Hospitalafter resection of gastric carcinoma. Statistics of the three major nutrients researchobject, according to the non protein calorie calculation formula to calculate theaverage non protein calorie content. According to the non protein calorie contentwere divided calorie supply to four groups:<15kcal·kg-1·d-1group、 caloriesupply (15-20) kcal·kg-1·d-1group, calorie supply (20-25) kcal/kg-1d-1groupand calorie supply>25kcal·kg-1·d-1group. Compared the differences of fourgroups of patients on the1day and4day after operation of serum albuminconcentration (ALB)、blood glucose、serum ALT and AST, postoperative body weightchange, hospital stay and cost of hospitalization.Results:1.In study78patients met the criteria of patients, including40cases of male,female38cases, mean age56.03±7.131years old; according to the amount ofcalories subjects were divided into four groups, four groups of patients the averagenitrogen content in0.08-0.1g.kg-1.d-1, no significant difference between the fourgroups in gender, age, weight, preoperative intestinal the average amount of nitrogennutrition and postoperative albumin, albumin after input amount.(P>0.05)2. Four groups of subjects in the preoperative serum albumin (P=0.77) and the1postoperative day serum albumin (P=0.257) showed no statistically significantdifference; And albumin levels in plasma after4days there are differences between the fourgroups (P=0.002), Plasma albumin mean on the4day after operation in the fourgroups were multiple comparison: Albumin level of the first group,the secondgroup,and the third groups difference.Plasma Plasma albumin of first group werelower than the second、 third group.And second groups comapared with third waswith no significant difference between them.The second group,the third group andforth groups serum albumin level is higher than the first group.3. The first days and the fourth days after operation has a increased tendency.Butthe result of plasma ALT and AST of Four groups before operation, the first days andthe fourth days after operation, show no significant difference between first groups inpreoperative, the first day and the fourth days after operation (P>0.05).4. Glucose has no significant difference between the four groups beforeoperation (P=0.269), and after first days and fourth days after operation, bloodglucose was statistically significant (P<0.05). Multiple comparisons of blood glucosewere performed on the first postoperative day and fourth days after operation, thecomparison results show: after first days and fourth days of blood glucose level, therewas no significant difference in the first group and second group, while the secondgroup and the third group and the fourth group had significant difference in bloodglucose between the four groups. and it is increasing in turn with the caloriesincreasing.5. The four groups had no significant difference on the total infection complicationsafter operation.6.Four groups of subjects to reduce weight in weight (P=0.132), thehospitalization time (P=0.405) and hospitalization expenses (P=0.802) had nodifferences, but the mean body weight changes visible on the first group of weightloss than other groups slightly higher.Conclusions:This study shows that the energy supply, caloric parenteral nutrition given15-20kcal·kg-1·d-1meet the physiological needs of Patients Undergone radical resectionfor carcinoma of stomach, the improvement and high caloric energy parenteralnutrition group and the15-20kcal·kg-1·d-1caloric energy parenteral nutrition group, which can not only meet the physical needs, but also to avoid the metabolic disorders,liver function damage in hyperglycemia input more nutrients cause and may cause.
