Objective:To investigate the main factors predicting functional outcomes ofkindey after Retropertioneal Laparoscope Nephron Sparing Surgery(RLNSS),detecting RLNSS surgical safety, and provide theoretical for impoving RLNSSsurgical procedure. Methods:Form October2011to December2013,54patients underwentRLNSS,Glomerular filtration rate(GFR)of the renal units with tumor form99mTc-Diethylenetetramine pentaacetic acid(99mTc-DTPA) renal scan was measuredpreoperatively and3months postoperatively.Patient, tumor and perioperativecharacteristics with respect to changes in GFR of operated kidney had beenanalyzed.Statistical analysis was performed using the Student t test、ANOVA andmultivariate linear regression analysis.Results:54cases of renal tumors were successfully completed RLNSS surgery,No one laparotomy surgery.Mean of tumor sizes was1.5-7.2cm.(4.54±1.53).Operation time35-120min (57.09±14.09).wit0-42min(23.06±6.77.)Estimate theamount of bleeding30-250ml (74.10±48.93).Mean preoperative GFR andpostoperative GFR after3months were24.20-62.32(46.50±9.62)ml/minand11.23-57.45(28.79±12.71)ml/min, respectively.Reduction and percent reduction inGFR were (17.70±7.04) ml/min and38.06%,respectively.On multivariateanalysis,Preoperative GFR of operated kindey(p=0.048)、Tumor size(P=0.001)、WIT(p=0.012)were significant. Conclusion:Preoperative GFR of operated kindey, tumor size and WIT were themain facters influencing renal function after RLNSS. |
