Objective:To explore the clinical value of256-slice CT perfusion (CTP) imaging in thepatients who were diagnosis of aneurysmal subarachnoid hemorrhage (SAH) andpostoperative cerebral vasospasm (CVS), and using CTP combined with CT angiography(CTA) to determine the patients’s prognosis who were diagnosis of aneurysmal subara-chnoid hemorrhage(SAH) and postoperative cerebral vasospasm(CVS).Methods:The68patients were divided into two groups according to the time ofoccurring SAH.(acute phase group≤3d, delay phase group4~15d).Then the patientswere divided into two groups (preoperative and postoperative groups) according towhether occurring CVS or not The postoperative patients were divided into early group(<3d) and late group (3~14d) in accordance with the review time. All the patients werereceived256-slice CT scan, CTA check and CTP scanning. Using the volume rendering(VR) to reconstruct the image of cerebral artery time data and CTP parameter to evaluatethe cerebral blood flow changes after CVS. Using the cerebral blood perfusionparameters to assess the impact of patients diagnosis of CVS after SAH.Results:There is no significant difference in the patients with CVS between preoperativeand postoperative groups. The cerebral blood flow in the patient with CVS betweenpreoperative and postoperative groups changed obviously.The perfusion of regionalcerebral tissue shows reduce, meanwhile, with the CBF decreasing,and both MTT andTTP extending.The incidence of CVS in early postoperative patients (66.7%)wereobviously higher than the late ones(14.3%). There were difference of CBF, MTT, TTPbetween two groups, which suggest that the incidence of CVS is higher in early groupsof patients whose cerebral perfusion and cerebral microcirculation change moreobviously.Conclusion:The256-slice CT brain blood perfusion imaging could achieve a "one-stop"inspection by using a combination of morphological and functional learning technologyto provide dynamic cerebral angiography and whole brain perfusion imagessimultaneously which could effectively evaluate aneurysm CVS cerebral hemodynamicchanges after SAH. Besides, it could provide early diagnosis,appropriate treatment andexcellent clinical judgment of patients with SAH.
