Purposes1. To investigate the mechanism and related risk factors forpost-traumatic cerebral infarction in patients with severe traumatic braininjury;2. To guide the clinical reasonable assessment of the severecraniocerebral injury, scientific prevention and treatment of traumaticcerebral infarction.Objects&MethodWe retrospectively reviewed the clinical data of the paients with severetraumatic brain injury in department of neurosurgery,Shenzhen second people’shospital from January1,2010to December31,2013,from which the qualifiedcases of severe craniocerebral injury were selected,and then divided into PTCIgroup and non PTCI group according to the head CT examination results whetherthe occurrence of PTCI.The following factors were evaluated by univariate andmultivariate study: gender, age, Glasgow coma (Glasgow coma scale, GCS) score,on whether complicated with cerebral hernia, low blood pressure, high bloodsugar, subarachnoid hemorrhage, cerebral contusion, diffuse brain swelling,the fracture of the skull base and decompressive craniectomy. SPSS19.0statistical software was applicated for analyzing, univariate analysis byChi-square examination,multivariate analysis by Logisticregression.Significance level is a=0.05. ResultsBy univariate analysis, the differentces of these five risk factors, LowGCS scores, cerebral hernia, hypotension, subarachnoid hemorrhage, diffusebrain swelling, between the two groups was statisticallysignificant(P<0.05),they were important risk factors for PTCI.By multivariatelogistic analysis, Low GCS scre, cerebral hernia, hypotension were theindependent risk factors for PTCI.ConclusionsLow GCS scors, cerebral hernia, hypotension, subarachnoid hemorrhage,diffuse brain swelling are the important risk factors of PTCI, Low GCS scores,cerebral hernia, hypotension are the independent risk factors for PTCI. Ithas important clinical significance for the prevention and treatment of PTCIto focus on these risk factors assessment and reasonable intervention.
