Purpose The study was to adopt1H-MRS to observation and summarize thecharacteristics of Hepatic Alveolar Echinococcosis (HAE) metabolites’ waves andtheir metabolic characteristics. Then we can provide evidence for the diagnosis anddifferential diagnosis from the aspects of growth pattern and biochemicalcharacteristics of HAE.Material and Methods From December2012to January2014in the Affiliated Hospital of Qinghai University, we select23cases of HAEdiagnosed by laboratory tests, imaging studies and clinical methods,20cases ofhealthy volunteers (control group) and8cases of primary hepatocellular carcinoma(HCC) patients. All of them were performed3.0T MR abdominal conventionalsequence and spectroscopic imaging examination, in alveolar hydatid liver lesions,cancer lesions and normal liver parenchyma performed single voxel1H-MRS imaging.Then comparative analysis of the spectral differences and the height of the peakswere measured for each compound within the organization, and calculate the ratio ofCho/Lip2peak height.Results In23patients with HAE,3cases are removed dueto excessive noise or baseline instability affect the results.20cases included in thestudy actually. Liver lesions in patients in each group were single, each measuring2voxels lesions and normal control group also measured two voxels, the actualmeasurement of96voxels. In20patients with HAE, there are33voxels slightlyhigher Cho peak is detected in the40voxels survey area, the detection rate was82.5%(33/40), which has seven survey area detected by reverse Cho peak.40voxelsLip peaks and H2O peaks were detected, the detection rate was100%.20cases ofHAE40voxels have34survey areas’ Cho peak higher than normal liver tissue,6Chopeak areas measured below the normal liver tissue. Cho peak of20cases of HAE in 40of the surveyed area were significantly lower than primary liver lesion. In the threegroup subjects, a maximum of four metabolites Peak displayed in each spectral peakin the curve, which are as follows: choline (Cho), fatty methyl peak (Lip1), aliphaticmethylene peak (Lip2) and water peak (H2O). Four peaks appear not always constant,and with no other significant metabolite peak integration. Cho peak, Lip2peaks andH2O peaks are the main. This study demonstrates that in the three groups the Lip2peak height, pairwise comparison, the difference was not statistically significant (P>0.05), the peak height of the cho and the peak height ratio of Cho/Lip2, pairwisecomparisons, the differences were statistically significant (P <0.05).Conclusion1、Cho peak mild rise probability of20cases of HAE examination is82.5%(33/40);2、The cho peak of HAE was mildly higher than normal liver tissue, and significantlylower than primary liver tumor tissue area;3、HAE liver lesions and normal livertissue, HAE liver lesions and primary hepatocellular carcinoma tumor tissue areas allshowed increased Cho/Lip2peak height ratio of varying degrees, but the latter risehigher;4、Cho peak elevated and Cho/Lip2peak height ratio can provide a referencefor HAE diagnosis and differential diagnosis of primary liver cancer.
