Objective:1、To study the law of release of the inflammatory factors and the effects ofuncontrolled inflammatory reaction in the early period of scald via investigating thechanges of the pro-inflammatory cytokine IL-1, IL-6, IL-8, TNF-α and anti-inflammatory cytokine IL-10in the scalded rats.2、To study the regulation effects of astragalus polysaccharides on inflammatoryfactors in the early period of scald and the possible mechanisms of the astragaluspolysaccharides affecting to the expression of the inflammatory factors viainvestigating the changes of TNF-α, IL-1, IL-6, IL-8and IL-10in serum after thetreatment of astragalus polysaccharides in the scalded rats.3、To explore the method of regulating orderly release of inflammatory factorsafter scalded,aiming to provide new ideas for preventing the injuried of fuction ofhistocyte and organ,also for improving the therapeutic effects of burn infection andthe prevention and control of MODS in the early period of scald.Materials And Methods:120healthy SD rats (wighted220+15) from the first affiliated hospital ofMUNIVERSITY were included in this experimental,with a male-female ratio of50—50. The rats were randomly divided into three groups: sham scald group (n=40),scald control group(n=40),scald with astragalus polysaccharide treatment group(n=40). The adaptive phase for the rats last for a week.Then each rats were depilatedand anesthetized. The sham scald group were subjected to37℃water mist scaldedfor8seconds.Other groups were given the108℃steam scalded for8seconds,witha pressure of4Mpa, which could led to30%TBSA III degree scald. All the scaldedrats received anti-shock treatment immediately after injury. The wound were smearedwith iodophor twice a day.Then the sham scald group and scald control groupreceived an intraperitoneal injection of2ml of0.9%normal saline while theastragalus polysaccharide treatment group received an intraperitoneal injection of2mlof astragalus polysaccharide(25mg/ml).Subsequently, the same injection of each group were repeated at8am everyday.Observed and recorded the survival rate ofeach group.On days1,3,7,14after scalded,10rats from each group were anesthetizedfor getting6ml of the abdominal aortic blood in aseptic conditions.The blood wascollected in the procoagulant tube at room temperature for30min, then collect theserum after centrifuged at1000*g for15minutes.At the end of the experimentalprieod,the levels of TNF-α,IL-1, IL-6, IL-8and IL-10in serum were measured byELISA.Results:1、Comparison of the survival rate of rats in each experimental group:the spirit,diet,activity, sensitivity, reaction, urination and defecation of each rats in sham groupwere normal;The rats of scalded control group showed low-spirited,the hair of itwas unkempt、 ruffled and not smooth, the back become slightly arched,breathedrapidly,lack of alertness, inappetency,the volume of urine decrease and the color ofurine turn yellow, frequency of defecation was significantly reduced and stool turndry at the first week.After a week the above symptoms had been gradually improvedand close to the sham scald group.Scald with astragalus polysaccharide treatmentgroup were low-spirited,inappetency and insensitive to stimuli on the first day,allaspects of the rats were close to the sham scald group on the third day.None of therats of sham scald group and scald with astragalus polysaccharide treatment groupdied in the experimental period.The rats of the scald control group after scaldedshowed that one death at the first and six day,two at the third and forth day.The rateof survival in the scald control group,sham scald group and scald with astragaluspolysaccharide group is85%,100%and100%, respectively.2、Comparison of each group of inflammatory factors IL-1, IL-6, IL-8, TNF-α,and IL-10:(1) The comparison of the scald control group and the sham scald group:tocompared with the sham scald group,the IL-1, IL-6, IL-8, TNF-α, IL-10weredecreased significantly (P<0.05) on the first day after scalded;on the third day,thelevel of IL-1and TNF-α reached the peak,then decreased gradually,and was nosignificantly different with the sham scald group on day14;The IL-6、IL-8increasedin volatility,reached the peak at day7,then decreased gradually,and were no obviousdifferent with the sham scald group on day14;IL-10rise in gradually,reaches peak at about7th day,then decreased rapidly,and finally the level of it was no obviouschanges between sham scald group on day14.(2) The comparison of the scald with astragalus polysaccharide treatment groupand the sham scald group:the level of IL-1, IL-6, IL-8, TNF-αand IL-10of the scaldwith astragalus polysaccharide treatment group were significantly decreased(P<0.05)on the first day after scalded;IL-1,IL-6, IL-8and TNF-αcompared with thatof the first day shows a declining trend though it still maintained at a high level,thenthe level go on declince until the it close to the sham scald group at day14;IL-10increased gradually after scalded,reached its peak on the third day,and wassignificantly diffierent with sham scald group (P<0.05),then it decreased gradually,and the level of them was no obvious diffierent with the sham scald group at day14.(3) The comparison of the scald with astragalus polysaccharide treatment groupand the scald control group:the IL-1, IL-6, IL-8, TNF-αof the scald with astragaluspolysaccharide treatment group were lower than the controls after scalded on the firstday. The difference between the groups is statistically significant(P<0.05).The levelof them statistically significantly decreased(P<0.05) on day3and day7,and finally itwas no obvious difference with the sham scald group on day14.The IL-10of the scaldwith astragalus polysaccharide treatment group was statistically significantly higher(P<0.05) than this of the controls on the first day after burned,reached its peak on thethird day,the peak also higher than the scald control group,and the level of it was noobvious difference with the control group on day14.Conclusions:1. Both the pro-inflammatory cytokine and anti-inflammatory cytokine hadexcessive released after the rats were severely scalded.2. Astragalus polysaccharide would not only reduce the generation ofpro-inflammatory cytokine IL-1、IL-6、IL-8and TNF-α,but also engender theanti-inflammatory cytokine IL-10after scalded in rats.In the early period ofscald,these changes were contributed to the regulation of the disorderly release ofinflammatory factor.
