Objective:Gastric cancer is a common malignancy of which accounts for about half of thetotal number of antral cancer patients with gastric cancer. Currently distal gastricresection is the primary means of their treatment, but surgery routine vagotomy,patients often appear delayed gastric emptying, reflux esophagitis, malnutrition andcomplications such as gallstones, seriously affecting the patient’s quality of life,research suggests that vagotomy surgery and postoperative gastrointestinal disordersare closely related. therefore, to investigate the efficacy of Reserved vagus nerve indistal gastrectomy surgery in the mid-antrum retained vagus has important clinicalbenefitsMethod:Select the First Affiliated Hospital of Nanchang University during the periodfrom February2012to February2013early and mid-antral cancer50patients.According to the sequence of operational time, divided into reserving vagus nervegroup (n=25patients) and the traditional group (n=25patients). Implemented inaccordance with the requirements of gastric D2radical distal gastrectomy, wereobserved in patients with intraoperative safety indicators, targets cancer cure,postoperative recovery of gastrointestinal function, gastrointestinal hormonefluctuations and nutritional status, and the patients were followed up for surgerysurvival and quality of life after a year.Result:1.Surgical safety indicators: there was no significant difference in operativetime、blood loss、 postoperative complications between the two groups (P>0.05);2.Indicators of tumor cure targets: two tumor margins were negative, and Thenumber of lymph nodes was no significant difference in two groups(P>0.05);3.Recovery of gastrointestinal function indicators: the gastrointestinal functionin reserving vagus nerve group have improved much more than traditional operationgroup (P<0.05);meanwhile postprandial abdominal pain and loss of appetite incidence was lower than the traditional group (P<0.05);4.Gastrointestinal hormone indicators: serum gastrin levels and serum glucagonlevels of the first seven days After surgery were significantly higher than the pre-operative levels in the traditional group (P<0.05),however,there was no significantdifference between the first seven days After surgery and the pre-operative levels inthe reserving vagus nerve group(P>0.05).5.Nutritional indicators: nutritional status of the reserving vagus nerve groupwere significantly higher than the traditional group (P<0.05);6.Prognostic indicators: there was no significant difference in1-year survivalrate after surgery t between the two groups of patients (P>0.05), but the quality of lifeof the reserving vagus nerve group was significantly higher than the traditional group(P<0.05).Conclusion:1.In terms of safety surgical technique: Reserved vagus nerve in distalgastrectomy is safe and feasible for mid-antral cancer patients;2.In terms of radical tumor: Reserved vagus nerve surgery can get the sameeffect with traditional remote surgical resection in gastric cancer cure;3.Postoperative recovery of gastrointestinal function:Reserved vagus nervesurgery can significantly promote the recovery of gastrointestinal function, reducepostprandial abdominal pain and increased appetite;4.In terms of gastrointestinal hormones: Reserved vagus nerve surgery cansignificantly promote serum gastrin levels and serum glucagon levels recover, makingit more in line with the normal physiological state;5.In terms of postoperative nutritional status:Reserved vagus nerve surgery cansignificantly improve the nutritional status of patients after surgery in the near futurefor;6.In prognosis, Reserved vagus nerve surgery does not affect patient survivalpremise, significantly improved the quality of life after the near future.
