Background:Women during pregnancy or postpartum have a higher risk of venousthromboembolism. There have been many studies to estimate the incidence of VTEduring pregnancy or postpartum, and the reported incidence was highly variable.Purpose: To obtain a relative accurate incidence of VTE during pregnancy andpostpartum, we conducted this systematic literature review and meta-analysis.Methods: We searched PubMed, EMBase, CNKI (China National KnowledgeInfrastructure), and Wanfang Data for all articles related to epidemiology of venousthromboembolism in pregnancy or postpartum. Then the identified items were underevaluation to determine which could be included in the analysis according to thewell-developed inclusion criterias. We calculated the pooled incidence of VTE, DVT,and PE and mortality of PE in R software. The pooled proportions of distribution ofVTE in different periods and different legs were undertaken using Meta analyst Beta3.13. We performed sensitivity analyses to check out the robustness of the results.Publication bias was assessed with the Egger’s test.Results:27articles met the inclusion criteria. The pooled incidence for VTE was1.4‰(1.0-1.8‰),1.1‰(1.0-1.3‰) for DVT and0.3‰(0.2-0.4‰) for PE. Themortality of PE was6.58%(3.30-13.09%). The weighted proportion for VTEpostpartum was57.5%(50.9-63.9%), and DVT in right leg was27.9%(20.7-36.4%).We noted substantial heterogeneity among individual studies. There wasn’tpublication bias in the analysis.Conclusions:The incidence of VTE in pregnancy and postpartum is higher than non-pregnant women of the same age and is associated with an increased risk ofmortality. Physicians should be of high vigilance to pregnancy-associated VTE.Anticoagulant prophylaxis and therapy should be given to women of high risk shouldor patients with conformed VTE. Background: Venous thromboembolism (VTE), including deep vein thrombosis(DVT) and pulmonary embolism (PE), is more likely to happen during pregnancy andthe puerperium. In fact, VTE has becoming the leading cause of maternal death indeveloped countries nowadays.Purpose: To determine the incidence of VTE during perinatal period.Materials and methods: This was a prospective cohort study. All participants wererecruited from Beijing Chaoyang Hospital during August2011and August2012.Eachparticipant was screened for DVT using Doppler sonography both before and afterdelivery.Results: Of the322participants, including174cases undergone cesarean section and148cases undergone vaginal delivery, only two developed DVT after cesareandelivery, giving an overall incidence of6.21‰. Neither of these two women hadadditional risk factors for VTE.Conclusions: The incidence of VTE during perinatal period is6.21‰, cesareansection would increase the risk. Physicians should be of high vigilance topregnancy-associated VTE, especially for women after cesarean section.
