Objective1. To examine the cell apoptosis and expression of permeability-glycoprotein170in peripheralblood mononuclear cell of children with primary nephrotic syndrome at different times.2. Analyze the relationship of cell apoptosis and expression of permeability-glycoprotein170and possible mechanism on the glucocorticoid (GC)resistance.Methods1.28children with PNS were elected and divided into two groups:steroid-senstive nephroticsyndrome (SSNS) group and steroid-resistance nephrotic syndrome(SRNS)groupdependingon their steroid response.10healthy children were chosen as control group.2. Blood samples of peripheral venous blood were collected respectively before GC and fourweeks after enough GC treatment. Then PBMCS were extracted from fresh andheparinizedblood by density gradient centrifgation.3. Flow cytometry(FCM) technique was applied to measure the cell apoptosis.4.Flow cytometry(FCM) technique was applied to measure the expression of P-gp170.5.Correlation analysis was performed between cell apoptosis and expression of P-gp170.Results1.The cell apoptosis of PBMCThere were no statistic difference in the apoptosis rate of PBMC among the three groups[6.14±1.85%,6.44±1.83%,5.56±1.53%respectively,P>0.05]before GCtreatment. Compared with before GC treatment, the apoptosis rate of SSNS increasedobviously[11.48±6.25%,P <0.01].But there was no notable change in the apoptosisRate of SRNS group[6.94±1.87%è P·0.05].2.Expression of P-gp170in PBMCBefore GC treatment,there was significant difference in positive expression rates ofP-gp170among the three groups[4.54±3.10%,11.78±4.52%,2.30±2.07%respectively,P <0.01]; the expression rate of SRNS was higher than those in other twogroups; the expression rate of SSNS was higher than that in control group. Fouf weeksafter GC treatment, the expression rate of P-gp170significantly increased both in SSNSgroup and SRNS group[7.29±2.69%,15.36±4.36%respectively,all P <0.01],and the expression rate of P-gp170in SRNS group was higher than that in SSNS group P<0.01.3. Correlation between cell apoptosis and expression of P-gp170There was no linear correlation between cell apoptosis and expression of P-gp170r=-0.174è P>0.05.Conclusion1.Both healthy and PNS children have expression of P-gp170,later is higher than fronter.2.The P-gp170high expression may be responsible for the GC resistance in PNSChildren.3.Delayed cell apoptosis may be involved in GC resistance.4. Apoptosis rate and P-gp170expression of PBMC may be served as the for assessingprognosis in PNS children.
