Object:The aim of this trial was to observe the medium and long term results for the hip joint function of patients who had been go through the total hip arthroplasty (THA) in our hospital.Method:We retrospectively visited61patients (70hips), who had been go through the THA with pairs of conical surface spiral mortar non-cemented prosthesis from Feb.2000to Sep.2006. The prosthesis was provided by BICON-PLUS of Switzerland. Those cases were received clinical follow-up by telephone. After that, the clinical results were scored by Harris method, and radiological results were evaluated by Delee&Charnley method with femoral side used Gruen classification to observe the radiolucent lines and osteolysis. The test data was analyzed using SPSS13.0.Results:61cases (70hips) were involved, but2cases were withdraw for death,22patients were lost to follow-up because of address changing and wrong number, and2of them refused us. Actually,35cases (38hips), including25male cases (28hips) and10female cases (10hips), the mean surgical age was54.5years (range was from35to68years), were received an average of9.1years review (range was from6to12years) with a follow-up rate of57.3%. In the cases obtained follow-up, the primary diagnoses contained avascular necrosis of femoral head (23cases), osteoarthritis (4cases), ankylosing spondylitis (2cases), post-THA prosthetic loosening (3cases), femoral neck fracture (2cases), and developmental dislocation of hips (1case). Clinical results:the average Harris scores were increased from27.7(range was from3to78.8) preoperative to85.2(range was from68to97.4) at follow-up, with an excellent-and-good rate of84.2%, including11excellent hips,21good hips,5fair hips,1poor hips. Radiological results:Osteolyses were observed in13hips with an occurrence rate of34.2%, including2hips (5.2%) of continuous radiolucent lines,11hips (28.9%) of regional radiolucent lines. But osteolysis was absented around the cup. There was1hip emerged clinical flexibility (2.6%) and1hip received revision surgery (2.6%).Conclusion:The study showed that the THA is an effective clinical therapy in treating severe hip diseases. It delivered fairly well clinical function and stability. Moreover, it had a high survival rate in the medium and long-term follow-up.
