Objective To assess the safety, feasibility and superiority of minimally invasivepercutaneous nephrolithotomy in treating upper urinary calculi, under the twelfth rib edgefor the end of before and behind renal calices.Methods Object selected by the Institute of Urology, Affiliated Hospital of HebeiUniversity implemented from June2010to November2011, in line with the case inclusioncriteria and exclusion criteria,80cases of percutaneous renal lithotripsy patients.64-row CTdata in patients with preoperative analysis and design on the CT safe puncture path, based onthe establishment of the channel path is divided into renal light path of39cases (experimentalgroup) after the group calyceal path41cases (control group) The two sets of minimallyinvasive percutaneous renal lithotripsy compare two groups of stone clearance rate, by thetime of percutaneous renal access was successfully established, the length of the percutaneousrenal access, blood loss, operative time, postoperative urine turned clear time differences inhospital stay and complications after surgery, data collection SPSS16.0statistical software,statistical process data are presented as mean±standard deviation, counting material usedchi-square test,, measurement data using independent samples t test, P <0.05was consideredstatistically significant.Results Careful study of preoperative patients with64-slice CT data, and draw the bestpuncture path of uniform of all patients to complete the surgery, the surgery went smoothly,does not appear large errors. Before light path percutaneous renal lithotripsy and light paththrough passing through the percutaneous renal lithotripsy in operative time was statisticallysignificant (P <0.05), less before the light path percutaneous renal lithotripsy surgery timethrough light path. Two groups of stone-free rate, by the time of percutaneous renal accesswas successfully established, the length of the percutaneous renal access, blood loss,postoperative urine turned clear time, postoperative hospital stay was the time difference isnot statistically significant (P>0.05).. There were no postoperative hemorrhage or peripheralorgan damage, serious complications such as renal pelvis perforation, all patients werefollowed up for3-6months, no ureteral stricture, residual stones, recurrent.Conclusions1.Twelve ribs, renal light path after the group calyceal path percutaneous renalaccess, line mPCNL deal with single kidney stones and upper ureteric calculi, are safe andeffective, a considerable efficacy. 2.The probability of renal light path line mPCNL blood loss and complications and didnot increase, but a clear stone effect, significantly shorten the operation time, relatively betterthan after light path.
