Objective: This paper discussed the long-term aerobic endurance exercise and drugs (metformin) intervention on the influence of the type 2 diabetic rats pancreas leptin - AMPK way in signal transduction and insulin resistance, so as to provide experiment basis for the molecular biology mechanism which drug intervention could improve the insulin resistance, also it could provide theoretical basis for type 2 diabetes drugs and exercise combination therapy in clinical treatment.Methods:1. The paper selected 77 SD male rats which were just weaning, and randomly divided these rats into seven groups: normal group; diabetes control group; exercise therapy group; exercise prevention group; exercise prevention and cure group; the drug therapy group, exercise and drug therapy group, each group had 11 rats. The rats of normal group were feed by normal forage, and the other six groups were feed by the high-fat forage, the experimental cycle was 12 weeks.2. After high-fat feeding in the past six weeks, the six diabetes model groups were intraperitoneal injected low doses of Streptozotocin (STZ) to cause type 2 diabetes. Before the type 2 diabetes molding, diabetes exercise prevention group and diabetes exercise prevention and cure group were trained six-week swimming exercise; after the molding, the exercise therapy group, the exercise prevention and cure group, and the exercise and drug therapy group were also trained six-week swimming exercise.The six-week drug therapy of oral perfusion were used to the drug therapy group and the group of exercise and drug therapy.3. The rats of each group were sampled at the end of 12th-week exercise, and some index were measured, such as blood sugar, insulin, serum index content, pancreas leptin and its receptor protein expression, and AMPKa Hushais protein expression.Results:1. Compared with normal group: the weight, blood sugar, insulin of diabetes control group were significantly higher than normal group (P﹤0.05, P﹤0.01, P﹤0.01). The serum of TC, TG, FFA were significantly increased (P < 0.01) (P < 0.01) (P < 0.01). The pancreas leptin of diabetes control group was significantly increased (P < 0.01), leptin receptor protein content was significantly declined (P < 0.01), AMPKa1 content and AMPKa2 content were significantly declined (P < 0.01).2. Compared with diabetes control group: the leptin content of exercise therapy group was decreased(P < 0.05), the trend of leptin receptor content was rising, AMPKa1 content was significantly increased (P < 0.01), AMPKa2 content was significantly increased (P < 0.01); the trend of leptin and receptor content of exercise prevention group was down, AMPKa1 content was significantly increased (P < 0.01), AMPKa2 content was significantly increased (P < 0.01); the leptin content of drug therapy group was significantly reduced (P < 0.01), the content of leptin receptor was significantly increased (P < 0.01), AMPKa1 content significantly was increased (P < 0.01), AMPKa2 content was significantly increased(P < 0.01).3. Compared with exercise prevention group: the leptin and receptor content of drug therapy group was increased, there was no statistical significance. AMPKa1 content was increased (P < 0.05), AMPKa2 content trend was decreased, there were no obvious differences. Compared with the drug therapy group, the leptin content of exercise and drug therapy group was increased, the trend of leptin receptor content was rising, there was no statistically significance. AMPKa1 content was increased rise (P < 0.05), AMPKa2 content was increased (P < 0.05).Conclusion:1. High fat, high sugar diets combined the type 2 diabetic rats which were induced by small doses of STZ to get the result that the pancreas appeared leptin resistance (higher leptin receptor), and the resistance suppressed the activity of AMPK, also decreased the inhibitory action to isletβcells, so it is one of the reasons to cause the insulin resistance.2. Long-term aerobic endurance exercise and drug intervention could improve the pancreas leptin resistance to some extent, and made leptin receptor level increased, restored the activation of leptin to AMPK, increased inhibitory action of AMPK to isletβcells, eased the insulin resistance in body, enhanced the insulin sensitivity and reduced the blood sugar. So the group which could have functions of prevention, treatment, prevention and cure to type 2 diabetes was the group of exercise and drug therapy.
