Study On Extension Pattern And Mechanism Of Appropriate Health Technique In Gansu Rural Areas

Objectives To establish and perfect effective extension pattern and mechanism of appropriate health technique(AHT), which is more suitable for China and especially western rural areas, and improve continue development of extension of AHT.Methods To collect related literature of AHT, and summary advanced experience and shortcomings in the process of extending AHT in the tenth five-year plan. Using conventional registration data and field survey data from the eleventh five-year project, extension pattern and mechanism of AHT were established in Gansu rural areas, together with the actual situation and characteristic in Gansu.Results (1) The screening pattern of AHT was established, which included groups demand, profession accessibility, hardware support and techniques classification. The screening mechanism of AHT, with the combination of evaluation, admittance, revision and elimination of techniques, was formed. With the screening pattern, twenty AHTs were selected. They included ten western health techniques, five health techniques of Chinese Traditional Medicine, and five family planning techniques. (2) The trinity training pattern of AHT was established, which included experts network, centralism teaching, and classified training. The training mechanism, including fitting in speciality, reasonable teachers, training attendance, full steering and effect feedback, was formed. (3)The extension pattern of AHT was established, which included organization management system, extension and application system, and quality control system. The extension mechanism of AHT was formed. It was composed by unified management mechanism, social mobilization mechanism, coordination linkage mechanism, diffusion transfer mechanism, policy inclination mechanism, and constraint and encouragement mechanism. Extension and application of AHT achieved good effect in rural areas. The coverage rates of AHT reached up to 100%, 96.82%, and 77.72% in the county-level, township-level and village-level institutions, respectively. The rates of utilization of AHT were 94.74% and 100.00% in county-level and township-level institutions, and 64.02% in village-level institutions. Training techniques were given applied. 15194 passengers got appropriate technical services. The effective rate of AHT amounted to 86.44-99.69%.Conclusion The establishment of screening mechanism, with the combination of evaluation, admittance, revision and elimination of techniques, ensured the scientific and completeness of state source of AHT. The screening pattern guaranteed applicability and accuracy for the screening of techniques. The training pattern and mechanism ensured training effectiveness and quality, and guaranteed effective transfer and diffusion of techniques. The extension pattern and mechanism made extension and application of AHT obtain good achievements, and laid the foundation for continuous and effective extension of AHT in rural areas.
