Objective:This study is to investigate whether the addmition time in domestic ICU patients will affect the mortality through retrospective analysis of cases with APACHEII score as the assessment tool, and to analyze the relative reasons and countermeasures so as to improve the relevant understanding of hospital management and clinical staff.Methods:A retrospective case analysis was carried out in patients who were first admitted to and discharged from ICU of affiliated hospital of Qingdao University from2001to2012. According to the inclusion criteria,1181patients were collected.Then they were divided into5groups (11~15,16~20,21~25,26~30,31~35) according to the APACHEII score to exclude the impact of severity of the disease on the prognosis within group.We defined the legal working hours as Monday to Friday from8:00to17:00and non-statutory working hours as Monday to Friday from17:01to7:59,any time at Saturday and Sunday as well as holidays. In each score group,we defined patients admitted in non-statutory working hours as observation group (537patients),and in legal working hours as the control group (644patients).We collected the total,the death and the living number. Then we calculated the mortality and the odds ratio and actualized statistical analysis.Results:Of the1181patients,758were male and423were female.The average age was(59.5±18.4) years old. A total of838people survived, and343died. The OR is more than1in all groups,and the dangerous degree and the mortality is higher in observation group than in the control group. But in11~15and16~20group,there was no significant difference(P>0.05). While in the relatively high group21~25,26~30and31~35group, there was significant difference (P<0.05).This showed that patients with APACHEII score≥21who were admitted in non-statutory working hours had a higher mortality.Conclusion:The severity of illness of patients with the lower APACHE score is lighter. They may have better prognosis and the influence of time is smaller. For high APACHE II score group of ICU patients,different admission time will have an impact on mortality.Patients admitted in non-statutory working hours have a higher mortality. Corresponding measures should be taken to decrease the mortality in ICU patients and to improve the treatment efficiency of ICU.
