The Recent Effect Of The Unstable Complex Fracture Of Proximal Humerus Of The Middle-adult And The Old Treated By The Replacement Of Humeral Head And Splint

Objective: By observing the result of the patients suffering from the unstable complexfracture of proximal humerus who were treated by the replacement ofhumeral head and evaluating the therapeutic efficacy and clinicalsuperiority, The author wants to find a reasonable way to treat the unstablecomplex fracture of proximal humerus for the middle-adult and the old,thereby the pain can be reduced and the shoulder’s function resumes assoon as possible.Methods: In the part of literature study, The author systematically expatiates the relativeanatomy of shoulder, incidence, age factor and pathogenesis of fracture ofproximal humerus. In this part, the author also systematically retrospects thediagnosis, classification and several treatment measures at home or abroad.In the part of clinical study, the author collects in hospital patients from 2005to 2006, 26patients were divided into 2 groups. Treated group includes 11patients, they were treated by the way of replacement of humeral head, whilecontrol group includes the other 15 patients, they were treated by splint. Theauthor sets the 1、7、14 day and 1、2、3、6 month after treated as observingtime,at these times, the author observes the patients’ recovery and scores therecord by a given standard. After statistical analysis, the author evaluates thetherapeutic efficacy of the replacement of humeral head.Results: Via literature study, the author summarizes that the replacement of humeralhead as a new treatment used in clinic, receives a satisfying result. Thereplacement of humeral head is a reasonable way to treat the unstablecomplex fracture of proximal humerus for the middle-adult and the old. Theclinical study results: at the 7、14 day and 1 month after treated, the painstatistics shows P＜0.05, this demonstrates that in the pain efficacy of treatedgroup is better that of control group; at the 7、14 day after treated, thefunctional recovery statistics shows P＜0.05, this demonstrates that in thefunctional recovery efficacy of treated group is better that of control group; at the 7 day after treated, the range of motion recovery statistics shows P＜0.05,this demonstrates that in the activity recovery efficacy of treated group isbetter that of control group; at the 7、14 day and 1、2 month after treated, thetotal recovery statistics shows P＜0.05, this demonstrates that in the totalrecovery efficacy of treated group is better that of control group.Conclusion:The replacement of humeral head can relieve the pain as soon as possible,this is helpful for patients to do exercises in early time. The replacement ofhumeral head is a reasonable way to treat the unstable complex fracture ofproximal humerus for the middle-adult and the old.
