Purpose To explore the chanracteristics of brain tumor hemorrhage and non-tumor hemorrhage in Susceptibility Weighted Imaging (SWI); to compare the differantial dignostic value of contrast-enhanced magnetic resonance imaging (CE-MR^SWI and their combination for brain tumor hemorrhage and non-tumor hemorrhage.Materials and methods26cases of brain tumor hemorrhage and26cases of non-tumor hemorrhage confermed by pathology and clinical follow-up have been collected in First Clinical Hospital of Shanxi Medical University from December2011to December2013,including12cases of astrocytoma,11cases of matastatic tumor,3cases of oligodendroglioma,10cases of hypertensive intracerebral hemorrhage,3cases of simple cerebral hemorrhage,13cases of vascular malformation hemorrhage(including5cases of cavernous angioma hemorrhage,3cases of arteriovenous malformation,2cases of venous malformation and3cases of mixed vascular malformation hemorrhage). Compare the correct rate of diagnosis of the cause of cerebral hemerrohage for CE-MR> SWI and their combination.Results (l)Cerebral hemorrhage was formed into lesion like and nodular mass type.In cases of brain tumor hemorrhage,21cases were enhanced inhomogeneously;5cases of tumor were completely covered by hemorrhage,with edges slightly irregular enhancement in CE-MR.In cases of non-tumor brain hemorrhage group,10cases of hypertensive intracerebral hemorrhage and3cases of simple cerebral hemorrhage visible mild enhancement, similar to the shape and form of the hematoma.(2)The signal of hemorrhage of brain tumor was more than the mixed signal.In cases of non-tumor brain hemorrhage group,10cases of hypertensive intracerebral hemorrhage and3cases of simple cerebral hemorrhage were homogeneous relatively.3cases of arteriovenous malformation bleeding were remarkable for blood vessels flow void signal;5cases of cavernous hemangioma hemorrhage were characterized by "popcorn" sign, and "hoop" sign;2cases of venous malformation hemorrhage were seen in coarse venous drainage and multiple medullary veins,in which one case was mixed with a typical "sea snake heads" signs;One case of mixed vascular malformation bleeding visible both "popcorn" sign,"hoop" sign and thickening of drainage venous.(3)In cases of brain tumor hemorrhage,23cases showed tumor-associated vein in or around of lesions in SWI;3cases showed no tumor-associated vein in SWI.(4)Pairwise comparison performed among the differential diagnostic accuracy of CE-MR, SWI and their combined with bleeding on the brain tumor and non-tumor,CE-MR combined with SWI is better than the CE-MR and SWI method alone.Conclusion (1) SWI can display tumor-related vein, contributing to the diagnosis of brain tumor hemorrhage;(2) On the basis of MR scan, the combination of CE-MR and SWI can improve the accuracy of the differential diagnosis of brain tumor hemorrage and non-tumor brain hemorrhage.
