Objective In elderly patients with knee arthroplasty using dexmedetomidine holds pyrimidine auxiliary limb nerve block and epidural block,By recording perioperative hemodynamic changes in patients with and possible adverse reactions monitoring to make anaesthesia effect analysis, Provide new methods for clinical implementation of knee replacement anesthesia.Methods Choose our elective knee arthroplasty patients,150cases of unilateral, aged65-80, were randomly divided into three groups. Low concentration dexmedetomidine auxiliary nerve block group (group L, n=50), a high concentration of dexmedetomidine auxiliary nerve block (group H, n=50) and caudalanaesthesia group (group M, n=50), Observe the comparison of the three patients’ groups with anesthesia related time argument, The change of hemodynamic,The blockade of sensory and motor nerve, intraoperative anesthesia satisfaction, The number of antalgic requirements with patients after24hour and adverse reactions occur.Results Hemodynamic indexes (MAP and HR) L and H group, compared with M group relatively more stable (P<0.05); M group of sensory nerve block scope more widely than L and H group (P<0.05); The motor nerve blockade is that L and H group compared with M group are significantly longer (P<0.05)); Analgesic time than L, H and M group also obviously prolong; Intraoperative anesthesia satisfaction between three groups have no obvious difference (P>0.05);The need of analgesia after24hour M group significantly greater than the number of L and H group; Adverse reaction occurrence of L group was obviously less than H and M group (P<0.05). Conclusion A comparison of useing low concentration of dexmedetomidine auxiliary lumbar nerve block and with high levels of dexmedetomidine auxiliary lumbar nerve block and epidural block in comparison In elderly patients with knee arthroplasty, the former can achieve the same as the latter two anesthesia analgesic effect, and can make patients with intraoperative hemodynamic more smoothly, more durable and analgesic effects and significantly reduced the occurrence of adverse reactions.
