Objective: In order to investigate on the metabolism indexes of blood glucose andblood lipid of patients with type2diabetes in troops under the multi-level simplestructured management mode, and the related metabolism indexes of ordinary patientswho went to hospital by themselves. And to explore the significance of multi-levelsimple structured management mode to the control of disease.Methods: Firstly, investigate on those486army patients (including soldiers andmilitary families) who under the management of multi-level simple structure andclassify them as the observation group. Then, classify those398local outpatients as thecontrol group. Afterwards, evaluate the fasting blood glucose,2-hour postprandial bloodglucose, blood pressure, total cholesterol, triglyceride, high density lipoprotein and lowdensity lipoprotein, etc., according to the2010Chinese Diabetes Prevention Guide.Finally, analyze those data to identify whether there is a difference between the twogroups of diabetic patients.Results:1. Essential information: The observation group:486people, including421male and65female, age73.4±13.6years, course of disease13.1±9years. The control group:398people, including171male and227female, age67.5±10.7years, course of disease13.8±8years.2. The analytic results of blood glucose: The observation group: FBG6.55±1.74mmol/L,2hPG9.65±2.81mmol/L, HbA1c7.22+2.29%, the reaching standardrate of these three indexes were67.5%,28.8%,52.7%; the control group: FBG7.96± 2.41mmol/L,2hPG11.82±3.02mmol/L, HbA1c8.16±2.69%, the reaching standardrate of these three indexes were43%,28.1%,41.2%. As a result, the level of fastingblood glucose,2hour postprandial blood glucose and the percentage of glycatedhemoglobin in observation group were lower than the control group, and the reachingstandard rate was higher than the control group(P<0.01).3. The analytic results of blood pressure: The observation group: SBP136.43±19.01mmHg, DBP77.28±11.63mmHg, the reaching standard rate of the two indexeswere57%,68.3%, and ratio of both SBP and DBP were reaching the standard was up to47.5%; the control group: SBP148.02±18.22mmHg, DBP76.56±10.76mmHg, thereaching standard rate of the two indexes were41.2%,64.1%, and ratio of both SBP andDBP were reaching the standard was30.4%. As a result, the systolic blood pressure ofthe observation group was lower than that of the control group, and the reachingstandard rate was higher than that of the control group; the levels of systolic bloodpressure and reaching standard rate have no statistical significance; ratio of both SBPand DBP were reaching the standard of the observation group was higher than that inthe control group.4. The analytic results of blood lipid: The observation group: TC4.28±0.99mmol/L,TG1.57±1.32mmol/L, HDL-C1.14±0.32mmol/L, LDL-C2.61±0.71mmol/L, thereaching standard rate of the four indexes were55.6%,68.5%,54%,44%; observationgroup: TC4.89±1.24mmol/L, TG1.61±0.99mmol/L, HDL-C1.02±0.41mmol/L,LDL-C3.04±1.04mmol/L, the reaching standard rate of the four indexes were51%,66.1%,53%,34.4%; The indexes of total cholesterol and low-density lipoproteincholesterol in observation group were lower than those of the control group, thereaching standard rate were higher than those of the control group; there was nostatistical difference in triglyceride and high density lipoprotein cholesterol.5. The analytic results of patient compliance and satisfaction: The observation group:the compliance rate of total drug therapy, oral therapy, insulin treatment were65.1%, 83.3%,40.9%; control group were46.9%,63.8%,21.4%. The observation group wasobviously better than the control group (P<0.01). The compliance rate of following thedoctor’s instruction which including diet control, unhealthy lifestyle changing, exercise,self monitoring of blood glucose, regular visits, prevention of complications was66.3%in the observation group and43.4%in the control group. The observation group wassignificantly higher than that in control group (P<0.01). Patient satisfaction whichincluding the communication between patient and doctor, knowledge of their disease,curative effect in the observation group were:90%,86.6%,82.1%; the control group:53%,51.3%and60.8%. The observation group was significantly better than the controlgroup (P<0.01).Conclusion:1. The multi-level simple structured management system which carried out by the AirForce Garrison Troops is conducive to the control of blood glucose, blood pressure andblood lipid metabolism in diabetics.2. The multi-level simple structured management system which carried out by the AirForce Garrison Troops is conducive to the strengthening of treatment compliance andsatisfaction in diabetic patients.3. The individual treatment and management system supervision and other aspects inthe multi-level simple structured management system which carried out by the AirForce Garrison Troops are still facing many problems and requiring improvement.
