Background:The periampullary carcinoma patients are on the rise with the elderly people growing.In the clinical work,doctor will make a treatment plan according to the situation of the patient,there are different viewpoint which operation should be chosen for the periampullary carcinoma patients because of no guidelines are available.Aims:I made a retrospective review of the prognosis to analyzed the perioperative data of the elderly (≥65years) ampullary cancer patients,evaluate the different influence of the two surgical options.Data Sources:From January2010to October2013,144patients with periampullary carcinoma were studied,45patients choosed the radical operation and the other choosed the double loop bypass operation. Data Extraction:I extracted the perioperative data from the144patients for analysis,including the morbidity,mortality and so on.Methods and Results:The two groups studied were comparable in terms of operative techniques.Operative times (221.91±70.766vs354.60±92.413min for PD and DLB,respectively), blood loss, postoperative complications, operation-related mortality and hospital stay were higher in the PD group. Length of hospital stay and operative time were the parameters which differed significantly, while PD showed a trend towards a higher rate of postoperative complications.In particular, the rate of pancreatic fistula (32.3%vs0%), Abdominal infection (29.3%vs11.1%) and pulmonary complications (25.3%vs8.9%) were elevated in the patients who underwent PD. In addition, the other postoperative complications rates were higher in the PD group, although the difference between the two groups was not statistically significant.Conclusion:Over-65-year-old patients choose DLB with little risk, safety, good efficacy and less complications etc.However, there was no significant difference in long-term survival rate.
