ObjectiveTo investigate the value of preoperative embolization ofmeningiomas.MethodsRetrospectivelyanalyze sixty-eight patients with meningiomas,which were admittedtoDepartment of Neurosurgery,Hospital of Guangxi ZhuangAutonomous Region,between January2010and September2013.Thirty-sixpatients did not process cerebral angiography and embolization before surgicaloperation in early days, in contrast,thirty-two underwent preoperativeembolization in later days,among which thirty-one were conducted the resectionof lesions.Observing the loss of blood、the surgical time、the extent of tumorresection、the differences of the GOS when leaveing hospital,meanwhilerecording the complications of embolization.Result There were respectively17cases embolized with PVA(150~350μm)aqueous suspension and15cases with glubran and iodinated oilsuspension. The percentage of tumor devascularization was estimatedbycomparing the pre-and postembolization tumor blush at DSA:5cases with slightdevascularization;9with moderate devascularization;11with extensivedevascularization; and7with complete devascularization.The volume of hemorrhage in the group of embolization is less than the control group(P<0.05); no difference between the two groups in surgical time(P<0.05);the extent of tumor resection is better in the treatment group(P<0.05); no difference between the two groups in GOS (P>0.05) when leaving hospital. Totally four complications of embolizationoccurred (12.5%),including onewhich undid the surgery, but all abovecomplicationgs could be relieved effectively after appropriate treatments.Conclusion The preoperative cerebral angiography can guide theintraoperative procedure to protect important blood vessels and deal with tumorvessels. The preoperative embolization of meningiomas can lessen thehemorrhage during surgery, and increase the total resection rate oftumor.However the complications of embolization can not be ignored.
