Objective To investigate the change of haemoglobin (Hb) level in patients with chronic obstructive pulmonary disease(COPD) and to survey the impact of abnormal Hb on COPD patients.Methods According to a study in chronic obstructive pulmonary disease diagnostic criteria and exclusion criteria,75patients with stable COPD in the Second Hospital of Lanzhou Unversity were enrolled in our study from Mar.2012to Jan.2014. Recorded the patient’s age,sex, height, weight, history of oxygen therapy concentration of hemoglobin (Hb), red blood cell (RBC), etc. Calculated the patient’s body mass index(BMI). Patients were classified as normal hemoglobin group (normal group), hemoglobin increased group(HB≧170g/dl and≧160g/dl for males and females)and hemoglobin decreased group(anemic group,Hb<120g/dl and<110g/dl for males and females). The prevalence of anemia in patients with chronic obstructive pulmonary disease was calculated. The modified medical research council dyspnea scale (mMRC),COPD Assessment Test (CAT)were performed for the survey in the study, pulmonary function and the six-minute walking distance test (6MWD) were applied to evaluate all the COPD patients,the BODE score was calculated.The above indicators were compared between groups.Results1.Anemia was present in9(12%) patients and hemoglobin increased in8(10.67%), hemoglobin normal in58(77.33%). According to the severity and red blood cell morphology,anemia in COPD appeared as mild normocytic and normochromic;2. Compared with mMRC,6MWD,BODE scores,there were significant differences between the anemic group and the others(normal and hemoglobin increased group), the anemic group as manifested by significantly higher mMRC (3.00±0.71) and BODE scores(6.00±1.32),mean6MWD was significantly shorter(228.55±19.74m).However,there were no significant differences between the three groups in BMI and FEV1Predicted. In the anemic group,the Hb levels were positively with6MWD(r=0.725,P=0.027),but negatively with mMRC,BODE scores (r1=-0.749, P1=0.02, r2=-0.801,P2=0.009).3.The mean CAT scores were significantly higher in anemic patients compared with the others.In the anemic group, the CAT score were27.66±5.83, while the hemoglobin normal group and hemoglobin increased group were17.69±9.03,18.13±4.05. The Hb levels were negatively with CAT scores (r=-0.857,P=0.003);4.In the group with hemoglobin increased, there were no significant differences between normal group in6MWD, FEV1Predicted,mMRC, BMI,BODE,CAT.Conclusion Anemia has highly prevalence in patients with chronic obstructive pulmonary disease,anemia may increase dyspnoea and reduce exercise capacity and also have negative impact on the quality of life in COPD patients, hemoglobin increased may have no association of the COPD patient’s clinical symptoms and the quality of life.
