Purpose:Hepatocellular carcinoma (HCC) is one of the world’s most common, witch isthe one of the tumor with high malignant degree, and the morbidity in the third place. Our country is one of the high morbidity area of HCC,its morbidity is about30.3percent of one hundred thousand,about140000people was died each year from HCC,accounts for over50%of HCC deaths around the world, which is a serious threat topeople’s health and life. Knight K’s study found that anemia is one of thecomplications of cancer, the morbidity is more than40%, especially in patients withchemotherapy drug treatment, the rate can be as high as90%. A number of studieshave indicated that anemia also can be secondary from the patients with HCC. FabianFinkelmeier,a foreign scholar found that the single measurement of hemoglobin couldpredict outcome of HCC patients, however, there is no related research in China. Thispaper further clear clinical features and prognosis of HCC with hemoglobin (Hb), redblood cell(RBC),red blood cell morphology parameters of average red blood cellvolume (MCV), red blood cell volume width (RDW) index of correlation analysis, todetermine the severity of the primary liver cancer illness and guide treatment, so as toimprove the quality of life in patients with primary liver cancer and prolong thesurvival time.Method:According to inclusion and exclusion criteria,collect206cases with HCC of ourGastroenterology,Hepatopancreatobiliary Surgery and Intervention in January2011-January2012inpatient with complete data, among which157were male, female49cases.The age of206cases range from38to67,the average age is51.Usingretrospective study,2-year follow-up period, analyze the correlation betweenhemoglobin, the form of red blood cell and characteristics, prognosis in patients withhepatocellular carcinoma.Grouped by gender, Barcelona Liver Clinic(BCLC) staging,Child-Pugh staging,3months,6months,9months,12months,15months,18months,21months,24months survival. Application of statistical methods to comparedifferent sex, different BCLC staging with HCC patients and different Chid-Pughstaging with the corresponding Hb, RBC,RDW, MCV index of correlation, and Hb, RDW index correlation with the prognosis of HCC for statistical analysis.Result:T test is used to calculate significantly statistical difference between differentgender of age (P <0.001), the results show that the incidence of HCC and death in theaged male patients is low. Further application t test to compare between groups withinthe Hb, RBC, MCV, RDW averages, the results showed no significant differencebetween hemoglobin levels in gender (P=0.074,0.229,0.086,0.056), present in thisstudy gender can not influence the above four indicators.Hemoglobin level and Child-Pugh, BCLC staging of HCC were significantlynegative correlation (r=-0.806, P<0.001; r=-0.799, P<0.001), RBC count andChild-Pugh, BCLC staging of HCC were significantly negative correlation(r=-0.723,P<0.001;r=-0.744,P<0.001),with increasing the BCLC staging of patients with HCC,numerical value with the decrease of hemoglobin level and RBC count. The higherstage of Child-Pugh,the lower hemoglobin level and RBC count.RDW is positively related with Child-Pugh staging (r=0.839,P<0.001), and alsothe BCLC staging is positively correlated (r=0.824, P <0.001); The MCV ispositively correlated with Child Pugh staging and BCLC staging (r=0.620,P<0.001;r=0.634,P<0.001), that is, as the higher Child Pugh score, BCLC stageing, the highervalues of RDW, MCV, and RDW is more sensitive than MCV.Hemoglobin is positively correlated with survival time, the lower hemoglobinvalue, the lower quality of patients’ life, the shorter the time to live; RDW isnegatively correlated with survival time, the higher RDW values the shorter survivaltime. Conclusion:Analysis of206cases of HCC patients showed:the onset age of HCC,male areyounger than female, and the male prognosis is poor; The lower hemoglobin level andRBC count, the heavier the severity of the HCC;The higher RDW and MCVvalues, the heavier the severity of the HCC; With the decrease of hemoglobin, and theincrease of the RDW, the quality of life on patients with HCC, survival rate isdecreased, present that hemoglobin and RDW can be used as one of risk factors that estimate the prognosis of HCC.
