Objective:At present domestic thyroid carcinoma has occupied the first place ofincidence of malignant tumor of head and neck. Its visible at any age, peakappeared in45to65years of age. The incidence is about3times in womenthan in men. Differentiated thyroid carcinoma (DTC) mainly includingpapillary carcinoma and follicular carcinoma. The occurrence of thyroidcancer is a complex process of more gene tamper.Its development is relevantwith oncogene activation, tumor-suppressor gene mutation and anti-apoptoticgene expression, but still do not find effective targets to control theproliferation and apoptosis. Apoptosis is an active cell death process whichtriggered by factors in vivo and in vitro cell death of stored in the program, theabnormal Apoptosis is significantly biological features of many malignanttumor, one of the reasons for malignant tumor cell proliferation is expressionimbalance of promoting Apoptosis genes and Apoptosis inhibiting gene.Although Survivin is the newly and minimum inhibition of apoptosis proteinfamily member discovered so far, but it has the most powerful anti-apoptoticeffect. A large number of Experiments prove its highly expressed in tumortissues and little expressed in adult group terminal differentiation, which isunique in all known apoptosis regulation. It is let the relationship betweenanti-apoptotic and cellular canceration more directly, thus become the idealtarget of research of diagnosis and treatment of malignant tumor. Thisexperiment by detecting Survivin expression in thyroid carcinoma and noncancerous tissue and cell apoptosis index, aimed to explore the role ofSurvivin expression in thyroid cancer occurrence, development and thesignificance.Methods:1Select the third hospital of hebei medical university patients thyroidtissue that through total thyroidectomy during2004-2013which clinical data are archived in department and complete, choose40cases with thyroid tissuethat unilateral gland leaves is thyroid carcinoma tissue and the contralateralgland is non cancerous tissue which reached by pathology diagnosis. Get40patients bilateral thyroid pathological tissue of80cases, the cancer tissue asthe experimental group, the contralateral carcinoma group as control group.To review all specimens by the high qualification pathologists. Has not beenall pathology are starting, preoperative radiotherapy, chemotherapy andendocrine therapy.2By adopting the method of immunohistochemical detection of Survivin inthyroid carcinoma side lobes and side lobes of the expression. Double sidelobes was evaluated by TUNEL method of cell apoptosis index.3The data statistical analysis using SPSS13.0software package. Thecomparison of Survivin protein expression with X2test. The comparison ofapoptosis index using t test and rank and inspection. With P <0.05forstatistical significance.Result:1Immunohistochemical detection of Survivin protein expressed in differentthyroid lesions in the organization.Survivin in thyroid carcinoma and carcinoma tissues were expressed,Positive expression rate in thyroid carcinoma was60.00%, positive expressionrate in non carcinoma tissue was7.5%, the difference was statisticallysignificant (P <0.05). In thyroid carcinoma, papillary carcinoma, follicularcarcinoma and medullary carcinoma of Survivin positive expression rate of54.84%,66.67%and100%respectively.2Immunohistochemical detection of the expression of Survivin protein’srelationship with thyroid cancer clinical pathological factors.Survivin positive expression rate in thyroid carcinoma with different ages,different gender, there was no statistically significant difference between thedifferent pathological types (P>0.05). But is associated with lymph nodemetastasis, clinical stage, tumor size (P <0.05).3Survivin protein expression in thyroid lesions in the organization’s relationship with apoptosis index.Apoptosis index in thyroid carcinoma group is lower than the control group,the difference was statistically significant (P <0.05).Positive expression of Survivin thyroid carcinoma tissue apoptosis indexunder negative expression of Survivin thyroid carcinoma tissue apoptosisindex. statistically significant difference (P <0.05).Apoptosis index in the expression of Survivin protein positive control groupis lower than the Survivin protein expression of negative expression of thecontrol group, difference has statistical significance (P <0.05).Conclusion:1The expression of Survivin in thyroid carcinoma tissues was significantlyhigher than that of control group, may be as benign and malignant thyroidnodules index of differential diagnosis.2Positive expression of Survivin in thyroid carcinoma tissues and thyroidcarcinoma, lymph node metastasis, clinical stage, tumor size, can be used as areference index for thyroid cancer risk evaluation.3The apoptosis index in thyroid carcinoma tissue is lower than the controlgroup, suggest thyroid carcinoma tissue apoptosis sensitivity is lower than thecontrol group.4Positive expression of Survivin thyroid carcinoma tissue apoptosis indexunder negative expression of Survivin thyroid carcinoma tissue apoptosisindex, Survivin may inhibit normal thyroid cell apoptosis, participate in theoccurrence of thyroid cancer development.5Apoptosis index in the expression of Survivin protein positive controlgroup is lower than the Survivin protein expression of negative expression ofthe control group, Survivin may participate in reduce the sensitivity of benigncell apoptosis, promote the transformation from benign to malignant tissue.
